Detection of the seasonal patterns of healthy and unhealthy behavior could be helpful for designing individual and population health interventions programs. This study investigates the seasonal variation in sales of common types of products in Poland and Polish Google queries related to healthy behavior. Data of index sales from a large Polish retail store franchise, from January 2014 to August 2019, has been analyzed. The commercial data included twelve types of products. The interest of Google users was investigated using Google Trends statistics for the same period for six lifestyle-related topics. The seasonality was checked using time series analysis. Six of the consumer goods (dairy, ready-made meals, salty snacks, meats, beer, and cigarettes) were most commonly purchased in summer months, four (processed fish, food fats, wine, and alcohol 30%+) in December, and two (bread and sweets) in October. The lowest sales indexes were observed mostly in February. The interest in four topics that have been analyzed (“Diet,” “Dietitian,” “Weight loss,” and “Gym”), was highest in January, while interest in “Dietary supplements” was high in February, and “Running” in May. The search volume of the Google topics were the lowest in December. The purchase of food, drinks, and cigarettes, and the interest in information regarding different components of a healthy lifestyle has seasonal variation. New Year and Lent might be good periods to encourage healthy behavior. The motivation may decrease in summer and during Christmas. Background: There is a limited amount of data on the COVID-19 vector vaccine Gam-COVID-Vac (Sputnik V) safety profile. Previous infodemiology studies showed that social media discourse could be analyzed to assess the most concerning adverse events (AE) caused by drugs. Objective: We aimed to investigate mild AEs of Sputnik V based on a participatory trial conducted on Telegram in the Russian language. We compared AEs extracted from Telegram with other limited databases on Sputnik V and other COVID-19 vaccines. We explored symptom co-occurrence patterns and determined how counts of administered doses, age, gender, and sequence of shots could confound the reporting of AEs. Materials and Methods: We collected a unique dataset consisting of 11,515 self-reported Sputnik V vaccine AEs posted on the Telegram group, and we utilized natural language processing methods to extract AEs. Specifically, we performed multi-label classifications using the deep neural language model BERT DeepPavlov, which we pre-trained on a Russian language corpus and applied to the Telegram messages. The resulting AUC score was 0.991. We chose symptom classes that represented the following AEs: fever, pain, chills, fatigue, nausea/vomiting, headache, insomnia, lymph node enlargement, erythema, pruritus, swelling, and diarrhea. Results: The results of the retrospective analysis showed that females reported more AEs than males (1.2-fold, P<.001). In addition, there were more AEs from the first dose than from the second dose (1.13-fold, P<.001), and the number of AEs decreased with age (β = .05 per year, P<.001). The results also showed that Sputnik V AEs were more similar to other vector vaccines (132 units) compared with mRNA ones (241 units) according to the average Euclidean distance between the vectors of AE frequencies. Elderly Telegram users reported significantly more (5.6-fold on average) systemic AEs than their peers, according to the results of the phase III clinical trials published in The Lancet. However, the AEs reported in Telegram posts were consistent (Pearson correlation r=.94, P=.02) with those reported in the Argentinian post-marketing AE registry. Conclusion: After receiving the Sputnik V vaccination, Telegram users complained about pain (47%), fever (47%), fatigue (34%), and headache (25%). The results showed that the AE profile of Sputnik V was comparable with other COVID-19 vaccines. Examining the sentinel properties of participatory trials (which is subject to self-reporting biases) could still provide meaningful information about pharmaceutics, especially if only a limited amount of information on AEs is provided by producers.

Specifically, we performed multi-label classification using deep neural language model BERT “DeepPavlov”, pre-trained on Russian language corpus and tuned on Telegram messages. Resulting AUC score was equal to 0.991. We have chosen symptom classes representing AE: fever, pain, chills, fatigue, nausea/vomiting, headache, insomnia, lymph nodes enlargement, erythema, pruritus, swelling, diarrhea. RESULTS By retrospective analysis, we found that female report more AEs than males (1.2 folds, P<.001), there are more AEs in first than second dose (1.13 folds, P<.001) and No. AEs decreases with age (β = .05 per year, P<.001). Sputnik V AEs are more similar to other vector(132 units) than mRNA vaccines (241 units) by average Euclidean distance between vectors of AE frequencies. Elderly Telegram users report significantly more (5.6 folds on average) systemic AEs than their peers according to III phase clinical trial results published in Lancet. However, reported in Telegram AEs are consistent (Pearson correlation r=.94, P=.02) with Argentinian post-registration AEs registry. CONCLUSIONS Telegram users after Sputnik V vaccination complain mostly about Pain (47%), Fever (47%), Fatigue (34%) and Headache (25%) and the revealed adverse events profile of Sputnik V is comparable with other COVID-19 vaccines. Mining participatory trials of sentinel properties could provide meaningful information about pharmaceutics, especially if there is only limited information on AEs provided by producer with all limitation related to self-reporting biases (for instance local or gastric AEs could be under-reported as well as willingness to reporting satisfy typical product life-cycle temporal characteristics).

The Internet is a common source of health information as search engines propose websites that should answer users’ queries. The study aimed to investigate the search behavior of Google users related to skin clinical signs as well as to analyze their geographical, secular, and seasonal patterns. The data of Google Trends was used to analyze the number of Google searches related to skin problems from January 2004 to December 2019. Thirty-four topics representing dermatologic complaints were identified. The interests of all topics were calculated in proportion to the Relative Search Volume (RSV) of ‘Scar’. Geographical patterns as well as secular and seasonal trends were analyzed. Countries with few users who searched for skin problems were excluded from the analysis. Globally, gaining the most attention were ‘Itch’ proportion to RSV of ‘Itch’ (2.21), ‘Hair loss’ (1.56), ‘Skin rash’ (1.38), ‘Perspiration’ (1.32), and ‘Scar’ (1.00). In 42 of the 65 analyzed countries, ‘Itch’ was the most popular topic, followed by ‘Hair loss’ (n = 7), and ‘Pustule’ (n = 6). The RSV of all topics increases over time, with ‘Comedo’ (5.15 RSV/year), ‘Itch’ (4.83 RSV/year), and ‘Dandruff’ (4.66 RSV/year) being the most dynamic ones. For 23 topics, the highest interest was noted during warm months. Considering skin manifestations, Google users are mainly interested in itch, hair loss, and skin rash. An increasing number of individuals worldwide seem to use Google as a source of health information for dermatological clinical signs during the study period.

Twitter has considerable capacity for health education and proves to be an efficient and accessible communication tool in the coronavirus disease-19 (COVID-19) pandemic. Although many stakeholders saturate Twitter with COVID-19–related information, it remains unknown who disseminates information most efficaciously. COVID-19–related tweets were obtained from Twitter accounts of health agencies, governmental authorities, universities, scientific journals, medical associations, and celebrities. Posts' impact was measured with the nominal and relative (%followers) number of likes and retweets. A sentiment analysis was conducted.We have identified 17,331 COVID-19–related tweets posted by 338 accounts in >4 months since the virus began to spread. The largest number of likes was received by tweets of celebrities (median nominal, relative likes; 14,918, 0.036 percent), politicians (259, 0.174 percent), and health agencies (231, 0.007 percent). Most retweeted messages were also posted by celebrities (2,366, 0.005 percent), health agencies (130, 0.004 percent), and politicians (55, 0.041 percent). Retweets and likes peaked in March 2020, and the overall sentiment of the tweets was growing steadily. Whereas celebrities and politicians posted positive messages, the scientific and health authorities often employed a negative vocabulary. The posts with positive sentiment gained more likes and relative likes than nonpositive. During the pandemic, the tweets of celebrities and politicians related to COVID-19 outperform those coming from health and scientific institutions. Active engagement of Twitter influencers may help key messages go viral.

The reports suggesting a beneficial effect of nicotine on coronavirus disease 2019 (COVID-19) severity may encourage smoking. We aimed to analyze tweets on COVID-19 and smoking coming from casual Twitter users and Twitter accounts representing the tobacco industry. We collected tweets on COVID-19 and smoking from January 1 to May 1, 2020, using Twitter application programming interface. We analyzed sentiment, likes, or retweet to followers ratios, and the posts coming from the casual users to find pieces of news that could affect the discourse. Tweets coming from industry were analyzed manually. We analyzed n = 33,890 tweets on COVID-19 and smoking. The sentiment of tweets was negative, hitting a nadir in mid-March, but became less negative in April when preprints suggesting benefits from smoking on COVID-19 were released. Similar trends were observed for the ratios of likes or retweets to followers. We found 58 messages from the tobacco industry concerning COVID-19. Twenty-two (37.9 percent) mentioned the efforts of tobacco companies to support the development of a COVID-19 vaccine. Two tweets included Food and Drug Administration statements that there is no evidence that vaping increases the risk of COVID-19. The occurrence of preprints suggesting benefits of smoking in COVID-19 might increase sentiment and reactions to tweets on tobacco products and the virus. The authors of potentially controversial articles should restrain from the promotion of their results before the completion of the peer-review process. Twitter presents a convenient tool to monitor e-discourse during a health crisis. The research community should monitor the tobacco industry's social media.

The control of the coronavirus disease 2019 (COVID-19) pandemic depends on the profound investigation of the virus biology and its consequences. We aimed to analyze the COVID-19 research productivity of authors representing different countries and associations between the number of articles and COVID-19 spread. We retrieved all articles on COVID-19 indexed in PubMed between 31 December 2019 and 30 June 2020. We identified the countries of individual authors’ affiliations. We performed the R Spearman rank correlation test between the number of articles with at least one author from a country per one million citizens and Human Development Index (HDI), a number of COVID-19 cases and deaths per one million citizens before 1 July 2020. Overall, we identified 27,815 articles, including 18,225 original contributions, 2,449 reviews, and 69 meta-analyses on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. The highest productivity characterized the authors coming from China (n = 11,519 articles with at least one author), followed by the United States of America (n = 9,666) and Italy (n = 7,261). The number of articles on COVID-19 associated with HDI (Rs = 0.79), the numbers of cases (Rs = 0.47), and deaths (Rs = 0.46) (all p < 0.001). Early COVID-19 researches were most often authored by researchers from highly developed countries and those affected by the rapid initial spread of SARS-CoV-2.

We aimed to characterize the parenteral supplementation services in Czechia, Ireland, Italy, Poland, and the United Kingdom based on their websites. We generated a list of websites by searching Google using the term “vitamin infusion” and selected cities with 250,000 citizens from each analyzed country. All search inputs were performed using the native language. Data on the features of services, indications, contraindications, offered parenteral supplements, and social media activity were obtained. We analyzed 317 websites representing 371 active facilities. Only 6 (1.9%) facilities cited the scientific sources on parenteral supplementation, but these reference were highly biased; 17.4% did not provide information regarding their personnel, while 11.9% indicated the different contraindications. The most common indications were fatigue (62.5%), immunity enhancement (58.0%), anti-aging, and physical activity (51.5%). Approximately, 11.6% of facilities claimed that some parenteral supplements can help manage certain malignancies, while 2.2% claimed that they can help manage fertility problems. The most offered intravenous supplements were vitamins C (57.4%), B12 (47.7%), and B6 (42.3%). The parenteral supplementation market offers numerous ingredients as treatment for general health problems and serious health conditions. Many analyzed websites lacked essential information, which creates concerns for regarding the quality and reliability of the services.

Purpose: Colonoscopy is a gold standard for screening and diagnosis of colorectal cancer (CRC). The data from the search engine may reveal what information on coloscopy gains the attention of Internet users. We aimed to investigate Google searches trends and terms related to colonoscopy. Patients and Methods: We retrieved statistics searches related to colonoscopy using Google Trends (GT) and Google Ads (GA) for the period from April 2016 to March 2020. The GT data was used for the analysis of time and regional search patterns worldwide. GA data for Australia, Canada, Ireland, New Zealand (NZ), Poland, the United Kingdom (UK), and the United States (US) were used to calculate the search volume of categories of queries related to colonoscopy. Results: Globally, the relative search volume on colonoscopy has increased until the COVID-19 outbreak and revealed seasonal variation: the highest interest was observed in March (CRC awareness month), and the lowest during December (Christmas holidays). The highest number of searches per 1000 Google users-years was done in Poland (59.62) and the lowest in the UK (19.46). Most commonly, Google users searched for details on colonoscopy techniques (Australia, Canada, Ireland, NZ), anesthesia during the procedure (Poland), facility performing colonoscopy (UK, US). In all seven countries, less than 2% of queries concerned with bowel preparation before the procedure. Conclusion: Before the COVID-19 pandemic, the interest in colonoscopy has increased among Google users. Google users may underestimate the importance of proper bowel preparation.

We investigated Google queries in the years 2004–2019 as related to mode of birth methods in scope of global and local popularity, secular trends, and associations with real-world data. We retrieved data from Google Trends (GT) over time and regional interest in (n = 9) topics related to birth. We calculated the interest of all the topics in proportion to the Relative Search Volume (RSV) of ‘Caesarean section’”(CS). We retrieved WHO data on the CS rate and the World Bank data on the fertility rate. We analysed secular trends. Globally, the highest popularity was observed for these topics: ‘Childbirth’ (6.93 [times higher than CS]), ‘Caesarean section’ (1.00), and ‘Preterm birth’ (0.59). The regional RSV of ‘Caesarean section’ was associated with the real CS rate (r = 0.29; p = 0.016) and the interest in ‘Childbirth’ was associated with the regional fertility rate (r = 0.48; p < 0.001). Globally, the most dynamic rate of increase of interest was observed for ‘Vaginal delivery’ (+4.96 RSV/year) and ‘Caesarean section’ (+4.88 RSV/year), while a decrease was noted only for ‘Home birth’ (−3.04 RSV/year) and ‘Water birth’ (−1.84 RSV/year). The interest of Google users in most of the birth-related topics increased over time and is associated with real-world data. Using GT may provide insight into the interest of Google users regarding different birth-related matters. Health professionals should be active in e-discourse to provide reliable information and recommend trustworthy websites to women and those who support them.

The internet provides access to information about dietary supplements and allows their easy purchase. We aimed to rank the interest of Google users in dietary supplements and to determine the changes that occurred in their popularity from 2004 to 2019. We used Google Trends to generate data over time on regional interest in dietary supplements (n = 200). We categorized each included supplement and calculated the interest in all topics in proportion to the relative search volume (RSV) of “lutein”. We analyzed the trends over time of all topics and categories. Globally, the topics with the highest popularity were “magnesium”, which was 23.72 times more popular than “lutein”, “protein” (15.22 times more popular), and “iron” (15.12). The categories of supplements receiving most interest were protein (9.64), mineral (5.24), and vitamin (3.47). The RSV of seven categories of topics (amino acid, bacterial, botanical, fiber, mineral, protein, and vitamin) increased over time while two categories (enzyme and fat or fatty acid) saw a drop in their RSV. Overall, 119 topics saw an increase in interest over time, 19 remained stable, and 62 saw interest in them decrease. Google Trends provides insights into e-discourse and enables analysis of the differences in popularity of certain topics across countries and over time.

The Internet has become the main source of health-related information including nutrition. The aim of this study was to rank the most popular diets among Google users globally and regionally in addition to secular and seasonal trends in the years 2004 to 2019. We used Google Trends (GT) to identify and analyze course over time and regional interest of 47 topics related to diets. We analyzed secular trends using the Seasonal Mann–Kendall test and seasonal variation using time-series decomposition. The topic “Mediterranean diet” (MedD) was used as a benchmark. We calculated the interest of all topics in proportion to the relative search volume (RSV) of MedD. Globally, Google users were particularly interested in veganism (19.54 times higher than MedD), vegetarianism (15.09 times higher than MedD), and gluten-free diet (11.11 times higher than MedD). Veganism was the most frequently searched diet type in 23 countries followed by vegetarianism (14), ketogenic diet (7), and low-carbohydrate diet (7). Whereas an increase of RSV over time was observed for 23 diets, a decrease was noted for 20. The most dynamic increase was found for FODMAP (6.12 RSV/year), gluten-free diet (5.95 RSV/year), and raw veganism (5.72 RSV/year). Sharp declines concerned negative-calorie food (–4.34 RSV/year), macrobiotic diet (–3.89 RSV/year), and cabbage soup diet (–3.50 RSV/year). The interest in most diets falls in December but peaks in January. Veganism, vegetarianism, and gluten-free diet attract the largest public interest globally. Both secular trends and seasonal variation shape the ever-changing landscape of diet popularity. GT holds promise as a valuable tool in local and international nutrition research.

Internet forums are an attractive source of health-related information. We aimed to investigate threads in the gastroenterological section of a popular Polish medical forum for anonymous users. We characterised the following aspects in threads: the main problem of the original poster, declared ailments and rationale of the responses (rational, neutral, harmful or not related to the problem of the original poster). We analysed over 2717 forum threads initiated in the years 2010–2018. Users mostly asked for diagnosis of the problem [1814 (66.8%)], treatment [1056 (38.9%)] and diagnostic interpretation [308 (11.3%)]. The most commonly declared symptoms were abdominal pain [1046 (38.5%)], diarrhea [454 (16.7%)] and bloating [354 (13.0%)]. Alarm symptoms were mentioned in 309 (11.4%) threads. From the total 3550 responses, 1257 (35.4%) were assessed as rational, 693 (19.5%) as neutral, 157 (4.4%) as harmful and 1440 (40.6%) as not related to the user’s problem. The original poster’s declaration of blood in stool, dyspepsia, pain in the abdominal right lower quadrant, weight loss or inflammatory bowel disease was positively related to obtaining at least one potentially harmful response. Advice from anonymous users on Internet forums may be irrational and disregards alarm symptoms, which can delay the diagnosis of life-threatening diseases.

We aimed to rank the most common locations of pain among Google users globally and locally and analyze secular and seasonal trends in pain-related searches in the years 2004–2019. We used data generated by Google Trends (GT) to identify and analyze global interest in topics (n = 24) related to locations of pain and how these progressed over time. We analyzed secular trends and time series decomposition to identify seasonal variations. We also calculated the interest in all topics with reference to the relative search volume (RSV) of “Abdominal pain”. Google users were most commonly interested in “Headache” (1.30 [times more frequently than “Abdominal pain”]), “Abdominal pain” (1.00), and “Back pain” (0.84). “Headache” was the most frequent search term in n = 41 countries, while “Abdominal pain” was the most frequent term in n = 27 countries. The interest in all pain-related topics except “Dyspareunia” increased over time. The sharpest increase was observed for “Abdominal pain” (5.67 RSV/year), and “Toothache” (5.52 RSV/year). Most of the topics revealed seasonal variations. Among pain-related topics, “Headache,” “Abdominal pain,” and “Back pain” interested most Google users. GT is a novel tool that allows retrospective investigation of complaints among Internet users.

Half of the individuals with type 1 diabetes (T1DM) may present Vitamin D (VD) deficiency. There is little known about factors determining a decision on VD supplementation. The study aimed to determine the factors affecting vitamin D supplementation in people with T1DM. A cross-sectional survey study using the authors’ questionnaire paper and its digital version was performed. The questions involved data on the basic characteristics of the respondent, medical history, VD supplementation status, influence of the social environment, self-education, and the most important personal motivator for VD supplement use. Multivariate logistic regression analysis was performed. We collected a total of n = 184 papers and n = 550 digital complete surveys. From 734 total respondents, 62.0% declared VD supplementation. The main personal rationale for VD supplementation were recommendation of medical specialist 172 (37.8%) and self-education 135 (29.7%). The main reasons for non-supplementation of VD were lack of knowledge about VD 159 (57.0%) and lack of motivation 77 (27.6%). VD supplementation was independently associated with a family doctor (odds ratio (OR), 95% confidence interval (CI): 4.67, 2.32–9.40) or medical specialist recommendation (16.20, 9.57–27.43), and self-education (5.97, 3.90–9.13). Most Polish individuals with T1DM use VD supplements, and the decision is related to physicians’ recommendations and self-education.

Background: Internet traffic may reflect the prevalence of real-world ailments. The aim of this study was to analyse queries associated with abdominal pain and to investigate seasonal frequency and trends of searches in Poland. Methods: We analysed mean search volume from March 2015 to February 2019 of keywords associated with ‘abdominal pain’ in the Polish language generated from the Google Ads Planner. Keywords were categorised by location, feature or type of pain, concurrent symptoms, frequency and patient groups, etc. We compared differences in search volumes across seasons and years. Results: Overall, 24,673,430 queries were associated with abdominal pain in the period analysed. The number of searches associated with abdominal pain in winter increased by ~30% compared with queries searched in summer. There were significantly more searches related to abdominal pain located in the epigastric, umbilical, hypogastrium, left lumbar, left iliac and right iliac regions combined with vomiting or fever, and acute or burning sensation in cold months. We did not observe any specific patterns of pain in the right lumbar or left hypochondrium region, co-presence of flatulence, acute, cramping or persistent/chronic or strong abdominal pain. Conclusion: Internet queries associated with abdominal pain and related factors have increased in the past 4 years and present a seasonal pattern. Google Ads may be a valuable tool to assess the prevalence of complaints in under-researched regions. The observed trends in queries related to abdominal pain merely reflect the scale of the problem. The reported pattern should be verified in epidemiological studies.

Chronic idiopathic constipation (CIC) has emerged as common problem for contemporary gastroenterology and is one of the most frequent complaints in primary care. Chronic idiopathic constipation significantly affects patients' quality of life and has an impact on global health and economy. Functional gastrointestinal disorders and bowel disorders, according to Rome IV criteria, result from inappropriate gut-brain interactions. The pathophysiology is complex and poorly understood, with evidence accumulating that gut microbiota can be implicated in the development and function of the enteric nervous system. Gut bacteria modulate gut barrier function, short chain fatty acid synthesis, and bile acid metabolism, factors which play roles in the gut peristalsis regulation. The high prevalence of CIC, with poor treatment outcomes, warrants searches for new forms of therapy, including probiotic therapies. Probiotics are often recommended by medical practitioners, but evidence-based utility in adults with CIC is uncertain. Recommendations/guidelines are often based on results from individual studies, rather than meta-analyses or umbrella reviews. Additionally, meta-analyses often indicate a group of probiotics rather than individual strains, and they create difficulty for physicians in making therapeutic choices. More CIC patient randomised clinical studies utilising well-defined strains, or combinations, are necessary.

The internet is becoming the main source of health-related information. We aimed to investigate data regarding heartburn-related searches made by Google users from Australia, Canada, Germany, Poland, the United Kingdom, and the United States. We retrospectively analyzed data from Google Ads Keywords Planner. We extracted search volumes of keywords associated with “heartburn” for June 2015 to May 2019. The data were generated in the respective primary language. The number of searches per 1,000 Google-user years was as follows: 177.4 (Australia), 178.1 (Canada), 123.8 (Germany), 199.7 (Poland), 152.5 (United Kingdom), and 194.5 (United States). The users were particularly interested in treatment (19.0 to 41.3%), diet (4.8 to 10.7%), symptoms (2.6 to 13.1%), and causes (3.7 to 10.0%). In all countries except Germany, the number of heartburn-related queries significantly increased over the analyzed period. For Canada, Germany, Poland, and the United Kingdom, query numbers were significantly lowest in summer; there was no significant seasonal trend for Australia and the United States. The number of heartburn-related queries has increased over the past four years, and a seasonal pattern may exist in certain regions. The trends in heartburn-related searches may reflect the scale of the complaint, and should be verified through future epidemiological studies.
