Background: Bone mineral density (BMD) assessed by DXA is a well-established predictor of osteoporotic fracture risk. However, data regarding the Polish female population remains limited. Objective: To evaluate the predictive value of BMD measurements for vertebral, hip, and all low-energy fractures in women aged 50 years and older. Methods: A total of 1.311 women from the Małopolska region underwent BMD assessment at the femoral neck, lumbar spine. The average follow-up period was 10.2 years, during which 479 osteoporotic fractures were recorded. Results: DXA measurements at the femoral neck showed the strongest correlation with hip fracture risk. Each one standard deviation decrease in the femoral neck T-score increased the risk of hip fracture by 2.1 times (HR 2.10; 95% CI 1.28–3.46; p = 0.003), after adjusting for age, but is not linear. A 1 SD decrease in the hip T-score was associated with a 28% increase in the risk of all osteoporotic fractures (HR 1.28; CI 1.17–1.40; p < 0.001), 53% increase in vertebral (HR 1.53; CI 1.13–2.08; p = 0.006) and 30% in hip (HR 1.30; CI 0.81–2.09; p = 0.278). The AUC values for hip BMD and hip T-score had the highest predictive value—AUC (area under the curve was 0.732 and 0.720, p < 0.01). Conclusions: BMD at the femoral neck proved to be a stronger predictor of hip fractures than measurements at the spine, radius. The risk increase associated with BMD/T-score reduction was non-linear. These findings confirm results from other benchmark studies.
Systemic mastocytosis (SM) is a rare hematologic neoplastic disorder characterized by infiltration of various organs by clonal mast cells. It s characterized with cutaneous and organ involvement (hepatosplenomegaly, osteolytic changes, pathological fractures) and systemic symptoms related to the release of anaphylaxis mediators. Systemic mastocytosis was diagnosed in a 50-year-old patient based on WHO criteria. The disease course included fractures of the TH11 and L1 vertebrae, and densitometric tests revealed low bone mineral density (BMD). The patient was treated with risedronate for 3 years, yet BMD continued to decline. The therapy was switched to intravenous zoledronic acid, resulting in a significant increase in BMD in control tests and relief from pain. No new osteoprotic fractures were observed during the treatment. Keywords: mastocytosis osteoporosis treatment bisphosphonates

W pracy przedstawiono przypadek 39-letniej pacjentki skierowanej na oddział internistyczny z nieswoistymi objawami ogólnoustrojowymi i wysokim poziomem parametrów zapalnych we krwi.

Objawy obejmują silny ból brzucha, ból w klatce piersiowej, zaburzenia rytmu serca, wysypkę i zapalenie spojówek. Wyniki badań laboratoryjnych zwykle wykazują podwyższone markery stanu zapalnego, zaburzenia krzepnięcia i uszkodzenie serca. Leczenie obejmuje immunoglobuliny, glikokortykoidy i leki biologiczne w celu stłumienia nadaktywności immunologicznej, zmniejszenia stanu zapalnego i ochrony funkcji narządów. W niniejszym raporcie omówiono przypadek kliniczny 6-letniego chłopca z wysoką gorączką, bólem brzucha i wysypką. Rozpoznania różnicowe obejmowały chorobę Kawasaki, wstrząs septyczny i zespół wstrząsu toksycznego. Leczenie przebiegało zgodnie z wytycznymi WHO, wdrożono immunoglobuliny, glikokortykoidy i kwas acetylosalicylowy (ASA). Szybka interwencja zapobiegła powikłaniom, takim jak tętniaki wieńcowe i dysfunkcja mięśnia sercowego. Ciągłe badania i edukacja są niezbędne do ulepszenia strategii diagnostycznych i terapeutycznych.

Introduction and aim. Sepsis is a set of symptoms caused by the body’s uncontrolled inflammatory response to infection, leading to damage to more than one organ and homeostasis disorders. Symptoms of sepsis in children may be non-specific, making it difficult to diagnose quickly and effectively. Description of the case. This article presents the case of a 14-year-old patient who was admitted to the Paediatric Unit due to a high fever (up to 42 degrees Celsius) lasting from five days and severe pain in the area of the lower ribs on the left side, at the site of an injury that occurred two weeks earlier. Conclusion. In the case of unclear symptoms of high fever and indicators of inflammation, a broad diagnostic spectrum is necessary, including imaging and laboratory tests and specialist consultations. It is also important to consider the possibility of pulmonary congestion in case of postural pain and elevated levels of D-dimers. Regular patient monitoring and controlled laboratory studies are crucial in assessing the effectiveness of therapy and the progress of treatment.
