Acute heart failure (AHF) is a life-threatening condition with high morbidity and mortality. Even though this pathology has been extensively researched, there are still challenges in establishing an accurate and early diagnosis, determining the long- and short-term prognosis and choosing a targeted therapeutic strategy. The use of reliable biomarkers to support clinical judgment has been shown to improve the management of AHF patients. Despite a large pool of interesting candidate biomarkers, endothelin-1 (ET-1) appears to be involved in multiple aspects of AHF pathogenesis that include neurohormonal activation, cardiac remodeling, endothelial dysfunction, inflammation, atherosclerosis and alteration of the renal function. Since its discovery, numerous studies have shown that the level of ET-1 is associated with the severity of symptoms and cardiac dysfunction in this pathology. The purpose of this paper is to review the existing information on ET-1 and answer the question of whether this neurohormone could be a promising biomarker in AHF.

Distal radioulnar joint (DRUJ) instability is a complex condition that can severely affect forearm function, causing pain, limited range of motion, and reduced strength. This review aims to consolidate current knowledge on the diagnosis and management of DRUJ instability, emphasizing a new classification system that we propose. The review synthesizes anatomical and biomechanical factors essential for DRUJ stability, focusing on the interrelationship between the bones and surrounding soft tissues. Our methodology involved a thorough examination of recent studies, incorporating clinical assessments and advanced imaging techniques such as MRI, ultrasound, and dynamic CT. This approach allowed us to develop a classification system that categorizes DRUJ injuries into three distinct grades. This system is intended to be practical for both clinical and radiological evaluations, offering clear guidance for treatment based on injury severity. The review discusses a range of treatment options, from conservative measures like splinting and physiotherapy to surgical procedures, including arthroscopy and DRUJ arthroplasty. The proposed classification system enhances the accuracy of diagnosis and supports more effective decision making in clinical practice. In summary, our findings suggest that the integration of advanced imaging techniques with minimally invasive surgical interventions can lead to better outcomes for patients. This review serves as a valuable resource for clinicians, providing a structured approach to managing DRUJ instability and improving patient care through the implementation of our new classification system.

The triangular fibrocartilage complex (TFCC) is an intrinsic stabiliser of the distal radioulnar joint (DRUJ), which serves an important role in wrist movement, forearm rotation and grip strength. A stable DRUJ is a key requirement for pain‑free movement of the wrist. TFCC tears are a common ulnar‑sided wrist pain associated with decreased grip strength and impaired function. TFCC tears are classified according to the location and cause. The Palmer classification delineates tears into traumatic type 1 and degenerative type 2. The present study investigated the clinical outcomes of using micro suture anchors in open repair in comparison with trans‑osseous TFCC repair. From July 2017 to July 2020, 51 patients underwent open repair of TFCC tear by hand and upper limb surgeons in Royal Rehabilitation Center at King Hussein Medical Center (Amman, Jordan). Stability of DRUJ and tenderness at the ulnar aspect of the wrist were evaluated by Disability of the Arm, Shoulder and Hand (DASH) and Modified Mayo Wrist Score (MMWS) pre‑ and post‑operatively. A total of 38 patients (27 male and 11 females) underwent open repair using micro suture anchors and 13 (7 male and 6 female) by trans‑osseous repair. A total of 51 patients in the follow‑up 6‑48 months had significant improvement in pain and range of motion. A total of 45 patients showed improved grip strength and were pain‑free; six patients showed improvement of grip strength and decreased pain. For the patients who underwent TFCC repair by micro suture anchors, there was an improvement of MMWS from 70 to 90%, and for those who underwent TFCC repair by trans‑osseous technique, MMWS improved from 65 to 85%. There was also significant improvement in DASH score post‑operatively with both techniques, from 60 to 15% in the trans‑osseous technique and from 70 to 15% in the suture anchors. Open repair of TFCC tears using either micro anchor or trans‑osseous repair led to pain‑free range of motion, improved grip strength, stable DRUJ and improvement in DASH and MMWS scores, with no significant differences in clinical outcome between techniques.

The management of acute heart failure (AHF) is becoming increasingly complex, especially in patients with multiple comorbidities. Endothelin-1 (ET-1), a vasoconstrictive peptide, is an important mediator of neurohormonal activation, endothelial dysfunction, and cardiac remodeling—key processes involved in the pathogenesis of AHF. The aim of our study was to evaluate the diagnostic and prognostic performance of ET-1 in multimorbid AHF patients, compared to established markers such as amino terminal pro B-type natriuretic peptide (NT-proBNP) and high-sensitivity cardiac troponin I (hs-cTnI). We conducted a single-center prospective study including 76 patients; 54 with AHF and 22 serving as controls. Upon admission, all patients underwent a comprehensive clinical, echocardiographic, and laboratory evaluation, including plasma ET-1 measurement using the enzyme-linked immunosorbent assay (ELISA) method. Receiver operating characteristic (ROC) curve and area under the curve (AUC) analysis were performed to assess the diagnostic and prognostic performance of ET-1 in comparison to NT-proBNP and hs-cTnI. ET-1 levels were considerably higher in AHF patients than in controls (p = 0.02), with an AUC of 0.954, showing comparable diagnostic accuracy with NT-proBNP (AUC = 0.997), alongside strong correlations with signs of systemic congestion, increased hospital stay, and ventricular dysfunction. ET-1 had the strongest predictive accuracy for in-hospital mortality (AUC = 0.781, p = 0.026), outperforming NT-proBNP and hs-cTnI. For 30-day mortality, ET-1 remained a reliable predictor (AUC = 0.784, p = 0.016). However, as the follow-up period extended to one year, its predictive power declined, confirming ET-1’s prognostic efficacy only for short-term outcomes. Moreover, ET-1 levels were not influenced by the presence of comorbidities, demonstrating its potential as an independent biomarker. Our findings support that ET-1 is a valuable biomarker for both diagnosis and short-term prognosis in the assessment of multimorbid AHF patients.

Objective: This study aims to evaluate and compare the biomechanical performance of two Kirschner (K) wire configurations—the intra-focal and interfragmentary techniques—for the fixation of dorsally displaced distal radius fractures. The study also assesses the impact of K-wire diameter (1.6 mm vs. 2.0 mm) on mechanical stability. Methods: Sixty fresh turkey tarsometatarsus bones were selected and divided into four groups based on the K-wire configuration and diameter used. Fractures were created at standardized locations, and each bone was stabilized using either the intra-focal also known as modified Kapandji (Ka) or interfragmentary technique. Mechanical testing, including axial compression and flexion tests, was performed to assess the biomechanical stability of each configuration. Results: The interfragmentary configuration consistently demonstrated superior biomechanical performance compared to the intra-focal technique. Specifically, the use of 2.0 mm K-wires resulted in significantly higher axial stiffness (13.28 MPa) and load at break (3070 N) compared to the 1.5 mm wires. Confidence intervals further supported the robustness of these findings. The interfragmentary technique, especially with thicker K-wires, provided greater load-bearing capacity and stiffness. Conclusion: The interfragmentary technique with 2.0 mm K-wires offers superior mechanical stability compared to the intra-focal technique, making it the preferred choice for stabilizing comminuted extra-articular distal radius fractures. These findings suggest that adopting this technique may reduce the risk of postoperative complications such as fracture displacement or malunion. Further research involving osteoporotic bone models and clinical trials is recommended to validate these findings in real-world settings.

Background: Adolescent pregnancy remains a major global public health issue, often linked to socioeconomic and educational disparities rather than biological immaturity. This study aimed to identify sociodemographic factors associated with adolescent pregnancies and to evaluate their impact on maternal and neonatal outcomes in a tertiary hospital in Northeastern Romania. Methods: A retrospective analysis was conducted at the “Cuza Vodă” Obstetrics-Gynecology Clinic Hospital, Iași, over two periods: 2013–2017 and January–October 2025. Records of 637 mothers aged <20 years were reviewed. Variables included age, education, prenatal monitoring, gestational age, delivery mode, neonatal outcomes, and obstetric complications. Statistical analyses were performed using SPSS v26, employing ANOVA, Welch ANOVA, and post hoc tests (p < 0.05). Ethical approval was obtained from the institutional ethics committee. Results: The mean maternal age was 17.26 ± 1.5 years, with 82.6% from rural areas. Most had only primary or lower secondary education. Fully monitored pregnancies were associated with significantly higher birth weights (mean = 3249 g) compared with unmonitored pregnancies (mean = 3009 g; p < 0.001). Infants of mothers with low education had the lowest mean birth weights (2963 g; p = 0.002). Preterm births represented 14.3% of cases, and cesarean deliveries accounted for 34.5%. A slight but significant increase in maternal age was observed between 2013–2017 and 2025 (p < 0.001), suggesting delayed adolescent childbearing. Conclusions: Low educational attainment and inadequate prenatal monitoring remain major determinants of adverse neonatal outcomes among adolescent mothers. Comprehensive sexual education, improved prenatal care accessibility, and social support programs are essential to reduce adolescent pregnancy rates and improve reproductive health in Romania.
