Background/Objectives: Transcatheter aortic valve replacement (TAVR) has been introduced as an optimal treatment for patients with severe aortic stenosis, offering a minimally invasive alternative to surgical aortic valve replacement. Predicting these outcomes following TAVR is crucial. Artificial intelligence (AI) has emerged as a promising tool for improving post-TAVR outcome prediction. In this systematic review and meta-analysis, we aim to summarize the current evidence on utilizing AI in predicting post-TAVR outcomes. Methods: A comprehensive search was conducted to evaluate the studies focused on TAVR that applied AI methods for risk stratification. We assessed various ML algorithms, including random forests, neural networks, extreme gradient boosting, and support vector machines. Model performance metrics—recall, area under the curve (AUC), and accuracy—were collected with 95% confidence intervals (CIs). A random-effects meta-analysis was conducted to pool effect estimates. Results: We included 43 studies evaluating 366,269 patients (mean age 80 ± 8.25; 52.9% men) following TAVR. Meta-analyses for AI model performances demonstrated the following results: all-cause mortality (AUC = 0.78 (0.74–0.82), accuracy = 0.81 (0.69–0.89), and recall = 0.90 (0.70–0.97); permanent pacemaker implantation or new left bundle branch block (AUC = 0.75 (0.68–0.82), accuracy = 0.73 (0.59–0.84), and recall = 0.87 (0.50–0.98)); valve-related dysfunction (AUC = 0.73 (0.62–0.84), accuracy = 0.79 (0.57–0.91), and recall = 0.54 (0.26–0.80)); and major adverse cardiovascular events (AUC = 0.79 (0.67–0.92)). Subgroup analyses based on the model development approaches indicated that models incorporating baseline clinical data, imaging, and biomarker information enhanced predictive performance. Conclusions: AI-based risk prediction for TAVR complications has demonstrated promising performance. However, it is necessary to evaluate the efficiency of the aforementioned models in external validation datasets.

Background. Primary adrenal insufficiency is an uncommon condition that manifests as nonspecific symptoms such as fatigue, weight loss, salt craving, and hyperpigmentation. Common cardiovascular presentations of AI are hypotension, arrhythmias, and syncope. However, acute heart failure is an uncommon presentation. Case Presentation. Here, a 26‐year‐old man was hospitalized with vasopressor‐resistant cardiogenic shock, which was finally attributed to an adrenal crisis. His past medical history was notable for Hashimoto’s disease, controlled with oral levothyroxine. Conclusion. AI should be considered among patients with cardiogenic shock who are unresponsive to conventional inotropes. Additionally, a history of autoimmune diseases may increase the suspicion of AI. Although the presentation of cardiogenic shock in a patient with undiagnosed AI is considered a rarity, delay in prompt treatment can lead to life‐threatening conditions.

Coronary artery bypass grafting (CABG) is one of the most performed cardiothoracic surgeries worldwide. The aim was to assess postoperative complications of patients undergoing CABG in the northeast of Iran. This was a 10-year, large-scale retrospective cohort study in Golestan Province. Kordkuy Heart Center of Amiralmomenin Hospital and Shafa Private Heart Center were the study settings, which were the only heart centers providing services to patients with cardiovascular diseases in Golestan Province. Adult patients of both genders who underwent isolated on-pump CABG procedures between 2007 and 2016 were included, and postoperative complications were extracted directly from patients’ hospital records. Out of 3720 surgeries performed, 3704 eligible patients were recruited, with a mean (standard deviation) age of 59.0 (9.8) years, of whom 73% were male. The postoperative complications included acute atrial fibrillation (AF; 8.2%), major bleeding (3.5%), heart failure (HF) (3.1%), pneumonia (2.0%), myocardial infarction (1.3%), acute kidney injury (1.0%), and stroke (0.3%). According to multivariable analyses, age (odds ratio [OR] 1.01, 95% confidence interval [CI] 1.001, 1.02; P .028), left ventricular ejection fraction (OR 0.97, 95% CI 0.96, 0.99; P .004), and preoperative β-blocker (OR 1.31, 95% CI 1.02, 1.68; P .034) were associated with new-onset AF. Cardiopulmonary bypass time (OR 1.003, 95% CI 1.001, 1.006; P .042) was the only associated factor with postoperative major bleeding. Chronic obstructive pulmonary disease (OR 6.73, 95% CI 3.98, 11.40; P < .001), preoperative β-blocker (OR 2.60, 95% CI 1.69, 3.97; P < .001), and preoperative angiotensin-converting-enzyme inhibitors (OR 1.56, 95% CI 1.04, 2.33; P .029) were the associated factors with postoperative HF. In conclusion, acute AF, major bleeding, HF, pneumonia, myocardial infarction, acute kidney injury, and stroke were postoperative complications observed in patients undergoing isolated on-pump CABG.
