A 48-year-old man was diagnosed Acute myeloid leukemia with FLT3 ITD positive and PML/RAR?- negative. Remission was achieved after induction and consolidation chemotherapy. Allogeneic hematopoietic stem cell transplantation was performed from his full matched sibling donor. He relapsed after 5 years and his bone marrow examination revealed PML/RAR?- positive Acute promyelocytic leukemia. t(15:17) was positive and FLT-3 ITD was negative. Cytogenetic and molecular analysis confirmed donor cell origin. Donor didn’t develop Acute promyelocytic leukemia.

Hereditary hyperferritinemia-cataract syndrome (HHCS) is a rare disease characterized by high serum ferritin levels, congenital bilateral cataracts, and the absence of tissue iron overload. HHCS is a rare disease characterized by high serum ferritin levels, congenital bilateral cataracts, and the absence of tissue iron overload. In this case report, we aimed to present a young female patient with a history of cataracts in herself and her family members. Our patient had the dilemma of iron deficiency anemia and hyperferritinemia. As in our patient, patients with HHCS apply to many outpatient clinics and have difficulty in diagnosis. For this reason, we prepared our case to raise awareness.
Background/Objectives: Cytomegalovirus (CMV) reactivation is a major cause of morbidity and mortality in allogeneic hematopoietic stem cell transplant (allo-HCT) recipients. Letermovir is used to prevent infections, but it is not available in many countries. Ganciclovir and valganciclovir carry a risk of bone marrow suppression. Foscarnet is frequently used as a bone marrow-sparing alternative; however, its clinical utility is constrained by nephrotoxicity. In this study, we aimed to evaluate the efficacy and renal safety of foscarnet and to identify factors associated with acute kidney injury (AKI) occurring during foscarnet treatment. Methods: This real-world, retrospective, single-center cohort study included 40 adult allo-HCT recipients treated with foscarnet for CMV reactivation between May 2022 and February 2025. Results: CMV polymerase chain reaction (PCR) negativity was achieved in 90% of patients, with a median time to PCR negativity of 10 (4–28) days. AKI occurred in 42.5% of patients, and 12.5% required hemodialysis. Foscarnet treatment was associated with a significant increase in serum creatinine levels and significant reductions in potassium, calcium, and magnesium concentrations (p < 0.05 for all). In univariable logistic regression analysis, myeloablative conditioning was associated with higher odds of AKI (OR 10.29, 95% CI 1.15–91.63; p = 0.037), while male sex showed numerically higher odds that did not reach conventional statistical significance (OR 4.28, 95% CI 0.96–19.01; p = 0.056). Conclusions: Among allo-HCT recipients treated with foscarnet, virological clearance was frequently observed, while AKI and electrolyte disturbances were common. These findings suggest that close renal and electrolyte monitoring may be warranted. However, given the retrospective, single-center design, small sample size, and absence of a comparator group, the findings should be interpreted cautiously and confirmed in larger prospective comparative studies.
Background/Objectives: Relapsed/refractory (R/R) B-cell non-Hodgkin lymphomas (B-NHLs) remain a major therapeutic challenge, particularly in patients with aggressive subtypes and limited treatment options after multiple lines of therapy. The ViPOR regimen, a multi-targeted combination of venetoclax, ibrutinib, prednisone, obinutuzumab, and lenalidomide, has demonstrated promising activity in early-phase studies; however, real-world data are limited. Methods: We conducted a retrospective, two-center study including patients with R/R B-NHL treated with the ViPOR regimen between January 2024 and April 2026. Clinical characteristics, treatment responses, survival outcomes, and safety data were analyzed. Results: A total of 14 patients were included, with a median age of 45 years and a median of 3.5 prior lines of therapy. Most patients had advanced-stage disease (71% stage IV), and 36% had primary refractory disease. The interim overall response rate (ORR) was 62%, including 31% complete response (CR) and 31% partial response. At the end of treatment, the ORR was 54%, with a CR rate of 54%. Radiotherapy was incorporated in selected patients with residual disease and contributed to response deepening. Six patients (43%) were successfully bridged to allogeneic stem cell transplantation, all in CR. Responses were notably more favorable in the activated B-cell (ABC) subtype compared to the germinal center subtype. The most common adverse events were neutropenia (57%) and diarrhea (36%), and the regimen demonstrated a manageable safety profile. During follow-up, 57% of patients remained alive. Conclusions: The ViPOR regimen demonstrated promising efficacy and acceptable safety in heavily pretreated R/R B-NHL, particularly in the ABC subtype. Its ability to induce deep responses and enable bridging to allogeneic stem cell transplantation highlights its potential role in real-world clinical practice. Larger prospective studies are warranted to confirm these findings.

Aims: Relapsed/refractory (R/R) acute lymphoblastic leukemia and lymphoblastic lymphoma (ALL/LBL) remain clinically challenging entities with limited effective salvage options, particularly in T-cell and early T-cell precursor (ETP) subtypes. Venetoclax has shown encouraging activity in preclinical and early clinical studies; however, real-world data are still scarce. This study aimed to evaluate venetoclax-based regimens in adult patients with R/R ALL/LBL treated at a single-center. Methods: This retrospective analysis included 13 adults with R/R ALL/LBL who received venetoclax-based therapy between 2019 and 2025. Clinical characteristics, prior treatments, cytogenetics, venetoclax dosing schedules, treatment combinations, response rates and survival outcomes were extracted from institutional records. Responses were assessed using bone marrow evaluation, multiparameter flow cytometry for measurable residual disease (MRD) and PET-CT for extramedullary disease. Overall survival (OS) and progression-free survival (PFS) were estimated using Kaplan-Meier analysis. Results: The median age was 30 years, and most patients had T-ALL/LBL (61.5%), including two with ETP-ALL/LBL. Patients were heavily pretreated (median 3 prior lines); 38.5% had relapsed after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Venetoclax was administered with various backbones, most commonly FLAG (46.2%) or nelarabine/asparaginase (23.1%). Venetoclax treatment duration ranged from 7–28 days per cycle, with shorter schedules frequently used in individualized regimens. The overall CR/CRi rate was 84.6%, including 38.5% MRD-negative CR. Complete metabolic response (CMR) was achieved in 3 patients with extramedullary disease. Both patients with ETP-ALL/LBL achieved deep responses (CMR or MRD negative CR), aligning with the biological venetoclax sensitivity of this subtype. Following venetoclax-based therapy, 7 patients (53.8%) proceeded to allo-HSCT. Median OS was 7 months and median PFS was 5 months, comparable to previously published real-world series. Toxicities were manageable; although Grade ≥3 cytopenias were universal, prolonged cytopenias were infrequent with 7-14-day venetoclax courses. Infectious complications included febrile neutropenia (84.6%), mucormycosis (7.7%) and invasive aspergillosis (7.7%). No patient discontinued therapy due to toxicity. Conclusion: Venetoclax-based regimens demonstrated substantial activity in a young, heavily pretreated R/R ALL/LBL population, yielding high CR/CRi rates, MRD-negative remissions and successful transition to allo-HSCT in over half of patients. Efficacy in the ETP-ALL/LBL subgroup and manageable toxicity with shorter venetoclax schedules highlight the practicality of individualized real-world use. These findings support venetoclax-based combinations as a promising salvage and bridging strategy in R/R ALL/LBL, warranting further prospective evaluation.
