Objective. To analyze the immediate and long-term results of denervation of facet joints for facet syndrome in the lumbar spine.Material and Methods. The immediate and long-term results of the treatment of 59 patients with severe pain in the lumbar spine who underwent facet joints denervation were studied.Results.On a MacNab scale, 39 (66.1 %) patients rated the treatment results as good, and 20 (33.9 %) as mediocre. According to the Nurick scale, the 2nd level results of treatment (improvement) were recorded in 55 (93.2 %) cases, the 3rd level ones (unaltered) – in 4 (6.8 %). The follow-up data were collected on 37 (62.7 %) patients from 1.7 months up to 1.5 years after surgery: 13 (35.2 %) of them rated the long-term treatment results as good, 8 (21.6 %) – as mediocre, and 16 (43.2 %) – as bad.Conclusions.Denervation of facet joints is an effective minimally invasive method for treating facet syndrome caused by spondyloarthrosis. It allows significantly reducing pain and improving the quality of life of patients in the early and long-term postoperative period.

The article highlights the history of Morton's neuroma study. The epidemiology, etiology, and main risk factors that act as a trigger in the development of this pathology are presented. The main pathogenesis links are revealed. Modern ideas about the development of the pathological process and the results of morphological studies are reflected, indicating that a pseudoneuroma is formed on the intermetatarsal nerve during microtrauma. Modern methods of diagnostics and criteria for making a diagnosis are listed. The role of CT, MRI, radiography, and ultrasound is shown. The clinical picture of the disease is presented. The measures for the prevention and conservative treatment of Morton's neuroma are described. The significance of therapeutic and diagnostic blockades is indicated, as consequent regression of the pain syndrome is a confirmation of Morton's neuroma in the patient. The diseases with which it is necessary to differentiate the described pathology are listed. The main methods of surgical interventions performed for Morton's neuroma are presented.

The article is devoted to the issues of etiology, pathophysiology and clinical manifestations of spinal stenosis of the lumbar spine. The possibilities of modern instrumental diagnostics of the disease are described, the differential clinical diagnosis of lumbar stenosis is presented. The paper presents a detailed classification of spinal stenosis. The modern therapeutic tactics of the disease are described, indications for the surgical treatment of the disease are given. Modern methods of surgical treatment of stenosis are briefly and schematically described: decompression (microsurgical and endoscopic), interspinous fixation, stabilization (dorsal and ventral) and epidural stimulation of the spinal cord. The data on the immediate and long-term results of surgical methods of treatment are presented. The authors also presented their experience in the treatment of spinal stenosis of the lumbar spine. The use of differentiated surgical treatment tactics in 85–90 % of cases provides good and excellent results in the treatment of lumbar stenosis.

Material and methods. Twenty-two patients with compression-ischemic neuropathy were included into the study. Low-level laser device “ALOK-1” was used for laser therapy. Its beam impacts tissues directly; its power is 1 mW, wavelength – 0.63 μm, spot diameter – 0.8 mkm.Results. Subjective, objective and instrumental findings became significantly better, if to compare with the pre-operative period.Conclusion. The developed technique has shown its good efficiency. The intraoperative laser therapy CIN patients decreased the severity of symptoms up to their complete regression; patients were highly satisfied with their postoperative outcomes.

Pain is one of the crucial issues of modern medicine. The high pain prevalence causes significant material and social losses. The main scientific and practical efforts are aimed at studying the issues of pain etiology, pathogenesis and therapy. As an effective measure to solve this problem, the idea of organizing a network of centers and clinics for the pain treatment was put forward. Purpose of the work: description of the structure of the organization of multidisciplinary clinics and pain treatment centers. There are currently few publications on the organization of pain relief. The article discusses the basic principles of organizing pain treatment centers and pain relief systems abroad. In this organization, attention is drawn to the principles of interdisciplinary interaction, the development of treatment tactics and the organization of work with a patient suffering from pain. Further, the paper presents the experience of organizing specialized pain relief care abroad. An example of the work of a pain center on the basis of a multidisciplinary hospital (Privolzhsky District Medical Center of the Federal Medical and Biological Agency of Russia) is given. The authors shared the structure and organization of this pain treatment center. They also presented the results of its work. Conclusion. Currently, there are no specialized pain centers in Russia. Now in many cities of Russia, there is a process of formation of pain relief care. However, this process faces a number of difficulties.
