This review explores the interconnection between precursor lesions of breast cancer (typical ductal hyperplasia, atypical ductal/lobular hyperplasia) and the subclinical of multiple organ failure syndrome, both representing early stages marked by alterations preceding clinical symptoms, undetectable through conventional diagnostic methods. Addressing the question “Why patients with breast cancer exhibit a tendency to deteriorate”, this study investigates the biological progression from a subclinical multiple organ failure syndrome, characterized by insidious but indisputable lesions, to an acute (clinical) state resembling a cascade akin to a waterfall or domino effect, often culminating in the patient’s demise. A comprehensive literature search was conducted using PubMed, Google Scholar, and Scopus databases in October 2023, employing keywords such as “MODS”, “SIRS”, “sepsis”, “pathophysiology of MODS”, “MODS in cancer patients”, “multiple organ failure”, “risk factors”, “cancer”, “ICU”, “quality of life”, and “breast cancer”. Supplementary references were extracted from the retrieved articles. This study emphasizes the importance of early identification and prevention of the multiple organ failure cascade at the inception of the malignant state, aiming to enhance the quality of life and extend survival. This pursuit contributes to a deeper understanding of risk factors and viable therapeutic options. Despite the existence of the subclinical multiple organ failure syndrome, current diagnostic methodologies remain inadequate, prompting consideration of AI as an increasingly crucial tool for early identification in the diagnostic process.

Breast cancer (BC) significantly impacts the quality of life (QoL) of affected individuals. This study, conducted at Colțea Clinical Hospital, Bucharest, aimed to assess the impact of organ failures and metastases on QoL in breast cancer patients using EORTC QLQ-C30 and EORTC QLQ-BR45 questionnaires and the survival rate to understand the clinical journey and the quality of life status in breast cancer patients. From January 2019 to October 2022, a prospective, observational study surveyed 874 patients, revealing 201 fatalities, 66 refusals, and 607 eligible participants. Results indicated statistically significant differences in various QoL aspects for patients experiencing heart failure, including physical functioning, pain, insomnia, global health status, and overall summary score. Kidney failure exhibited significance in physical functioning for QLQ-C30 and body image, sexual functioning, and endocrine sexual symptoms for QLQ-BR45. Respiratory failure demonstrated significant differences across multiple QoL domains. Patients with bone metastases reported lower physical functioning (p = 0.006) and increased pain (p = 0.002). This study has revealed an overall 5-year life expectancy of 68.8%, with survival rates of 93.8% for Stage I, 86.3% for Stage II, and 77.2% for Stage III breast cancer. Metastatic cancer patients have shown a 35.6% survival rate over 45 months, with a median survival duration of 36 months. A significant limitation of our study was the administration of the questionnaire only once, preventing us from quantifying the impact of specific treatment types on quality of life. This study emphasizes the necessity of using standardized QoL assessments in clinical practice from the initial presentation to ongoing follow-up.

We present the case of a 46-year-old patient who arrived at the emergency department with chest pain following an episode of vomiting. The diagnosis was Boerhaave syndrome and acute mediastinitis. Due to the prompt presentation and the location of the rupture in the lower esophagus, emergency surgical intervention was performed, including esophageal suturing, mediastinal drainage, and jejunostomy for feeding. Postoperatively, the patient was transferred to the intensive care unit for advanced monitoring and support. The patient’s condition improved favorably in the intensive care unit, allowing for extubation. Progress continued positively, and the patient was discharged 12 days postoperatively with a functional jejunostomy. At regular follow-ups, the patient’s recovery remained favorable, and the jejunostomy was removed 30 days after the surgery. This case highlights the importance of rapid diagnosis and early surgical intervention in Boerhaave syndrome, demonstrating the successful management of a severe condition through a multidisciplinary effort.

Acute aortic dissection (AAD) is a critical cardiovascular emergency marked by the rupture of the aortic intima, resulting in blood infiltration into the media and the formation of a false lumen. AAD incidence varies by area, emphasizing the need for better diagnostics and epidemiological investigations. Bucharest University Emergency Hospital’s Emergency Department conducted this retrospective cohort analysis from May 2021 to May 2023. We examined 26 Stanford Type A aortic dissection patients to establish in-hospital mortality and one-year survival rates. The primary objective was to analyze demographic, clinical, and paraclinical factors and their impact on patient outcomes. A total of 57.7% of the study group was male and had a mean age of 58.2 years, and 69.2% of patients had hypertension, indicating its importance as a risk factor. Acute chest discomfort was reported by 53.8%, neurological problems by 30.8%, and syncope or hypotension by 42.3%. CT angiography and transthoracic echocardiogram (TTE) confirmed the diagnosis and assessed dissection severity. Pericardial effusion (19.2%) and moderate to severe aortic regurgitation (26.9%) were notable. Management varied by dissection intensity and location. Emergency surgery was performed in 61.5% of patients within 24 h of diagnosis, resulting in a 12.5% in-hospital death rate. Conservatively managed patients had a 60.0% in-hospital death rate. Timely intervention is crucial, since the surgical cohort had an 87.5% one-year survival rate compared to 30% for the conservatively managed cohort. Acute renal damage (25%), protracted mechanical ventilation (31.3%), and advanced supportive care infections were postoperative sequelae. Conservative care exacerbated visceral ischemia (20%) and heart failure (10%). Advanced age and hypotension upon admission were independent mortality predictors, emphasizing the need for early risk assessment and personalized treatment. Multimodal imaging, timely surgical referral, and excellent postoperative care improve AAD outcomes, according to this study.

Background and Objectives: Trauma remains a leading global cause of preventable mortality, and outcome prediction tools are essential for both triage and resource allocation. Timely and effective medical response to trauma patients is of the essence. While there are already some widely accepted trauma scores, our aim was to build a more precise model tailored to our cohort as a framework for future research. Materials and Methods: A retrospective cohort of 91 patients was analyzed, using several clinical and paraclinical factors to build a logistic regression model that predicted 30-day mortality. Results: After adjusting for collinearity, our final multiple regression model, comprising systolic blood pressure, glycemia, urea serum levels and number of fractures, showed an excellent model fit (McFadden R2 = 0.682; AUC = 0.94) for predicting 30-day mortality. Systolic blood pressure was significantly associated with mortality (OR = 0.944, 95% CI: 0.920–0.969). In our cohort it seems to surpass the performance of The Trauma and Injury Severity Score (McFadden R2 = 0.56; AUC = 0.90). Conclusions: Commonly available clinical parameters could contribute to risk stratification, highlighting the prognostic importance of hemodynamic instability and metabolic response. This study was conducted on a small cohort as an exploratory analysis. Further research and validation based on multicentric cohorts are needed.

Immune thrombotic thrombocytopenic purpura (iTTP) is a rare condition with multisystem involvement that, in the absence of early recognition and prompt initiation of specific treatment, can be life-threatening. Nowadays, the diagnosis of this pathology represents a challenge in itself, determined by the association of several factors: the heterogeneity of the clinical picture, the frequent absence of all components of the classic pentad, as well as the overlap of neurological, cardiac, abdominal, or renal manifestations with those of much more common medical emergencies. This narrative review synthesizes the current data from the literature on the presentation of iTTP in emergency departments, the clinical and paraclinical elements that outline the clinical picture and support the clinical suspicion, the role of ADAMTS13 activity, and the usefulness of integrating the PLASMIC and French scores in current practice and in therapeutic decision making. The available data highlight the association between thrombocytopenia and microangiopathic hemolytic anemia as a central element of suspicion and support integrating organ involvement assessment, differential diagnosis, and early sampling for ADAMTS13 testing into the evaluation of patients with suspected iTTP. It should also be noted that in patients with high clinical probability of iTTP, the decision to initiate specialized treatment should not be postponed until the ADAMTS13 result is obtained. In this way, a dynamic approach, based on the corroboration of data obtained from the medical history, clinical examination, laboratory and diagnostic investigations, and probability assessment tools, facilitates early diagnosis and rapid access to specific therapy with a favorable impact on the prognosis.
