Context: The aims of the study are to compare characteristics of subacute thyroiditis (SAT) related to different etiologies, and to identify predictors of recurrence of SAT and incident hypothyroidism. Methods: This nationwide, multicenter, retrospective cohort study included 53 endocrinology centers in Turkey. The study participants were divided into either COVID-19–related SAT (Cov-SAT), SARS-CoV-2 vaccine–related SAT (Vac-SAT), or control SAT (Cont-SAT) groups. Results: Of the 811 patients, 258 (31.8%) were included in the Vac-SAT group, 98 (12.1%) in the Cov-SAT group, and 455 (56.1%) in the Cont-SAT group. No difference was found between the groups with regard to laboratory and imaging findings. SAT etiology was not an independent predictor of recurrence or hypothyroidism. In the entire cohort, steroid therapy requirement and younger age were statistically significant predictors for SAT recurrence. C-reactive protein measured during SAT onset, female sex, absence of antithyroid peroxidase (TPO) positivity, and absence of steroid therapy were statistically significant predictors of incident (early) hypothyroidism, irrespective of SAT etiology. On the other hand, probable predictors of established hypothyroidism differed from that of incident hypothyroidism. Conclusion: Since there is no difference in terms of follow-up parameters and outcomes, COVID-19– and SARS-CoV-2 vaccine–related SAT can be treated and followed up like classic SATs. Recurrence was determined by younger age and steroid therapy requirement. Steroid therapy independently predicts incident hypothyroidism that may sometimes be transient in overall SAT and is also associated with a lower risk of established hypothyroidism.

Background/Objectives: The antidiabetic effect of SGLT2 inhibitors (SGLT2-is) is based on their ability to increase glucose excretion through urine by inhibiting the kidney-resident SGLT2 protein. Euglycemic diabetic ketoacidosis (EuDKA) is an uncommon but potentially life-threatening adverse effect of these medications, which are notable for their antidiabetic, cardiovascular, and renal protective properties. This study aimed to clarify the impact of SGLT2-is on demographic, clinical, and biochemical characteristics in patients with DKA. Methods: A total of 51 individuals with a diagnosis of DKA were included in the trial; 19 of these patients were treated with SGLT2-is, while 32 were not. Patients diagnosed with DKA and treated with SGLT2-is were compared to those not treated with the medication in terms of clinical, biochemical, and laboratory characteristics. Results: The age of patients utilizing SGLT2-is was statistically considerably greater than that of non-users (p < 0.001). EuDKA was exclusively noted in the SGLT2-is cohort (p = 0.005). Urinary tract infections, vulvovaginitis, and genitourinary infections were substantially more prevalent among SGLT2-i users compared with non-users among both women and the overall patient group (p = 0.036, p = 0.001, p = 0.005, p = 0.003, respectively). Plasma glucose concentrations were significantly higher in SGLT2-i non-users (p = 0.006). Chloride (Cl−) concentrations were elevated among SGLT2-i users (p = 0.036). Conclusions: The study findings indicate that SGLT2 inhibitors may substantially influence age, serum chloride, EuDKA, and the occurrence of genitourinary infections in individuals with DKA.

Aims: Celiac disease (CeD) is an immune-mediated enteropathy with multisystem involvement that is often underdiagnosed due to variable clinical manifestations. Identifying reliable, accessible, and noninvasive biomarkers is essential for timely diagnosis, particularly in resource-limited settings. This study aims to evaluate the diagnostic utility of inflammation and nutrition-related indices and scores calculated from routine laboratory tests in predicting CeD. Methods: This retrospective cross-sectional study included 79 biopsy-confirmed celiac patients and 60 healthy controls. Demographic, hematological, and biochemical data were collected. The platelet-to-lymphocyte ratio (PLR), triglycerideglucose index (TyG), hemoglobin, albumin, lymphocyte, and platelet (HALP) score, and other inflammation-related indices were calculated via validated formulas. Logistic regression analysis was performed to identify independent predictors of CeD. Receiver operating characteristic (ROC) curves were used to assess diagnostic performance. Results: Compared with controls, patients with CeD had significantly greater PLRs and lower TyG indices and HALP scores (p

Objectives: T wave’s peak and end interval (Tp-e), Tp-e/QT ratio and Tp-e/QTc ratio are novel markers of ventricular repolarization and are associated with ventricular arrhythmias. Increased ventricular arrhythmia incidence is reported in patients with acromegaly. The purpose of this study is to evaluate ventricular repolarization using the Tp-e interval, Tp-e/QT ratio, and Tp-e/QTc ratio in patients with acromegaly. Methods: Thirty-five patients with acromegaly were included in the study. The control group was consisted of forty-one subjects without acromegaly that having similar age, sex ratio and comorbidities. The Tp-e interval, Tp-e/QT ratio, Tp-e/QTc ratio, and other ventricular repolarization parameters of all patients were evaluated using electrocardiography. Results: Tp-e interval, Tp-e/QT ratio and Tp-e/QTc ratio were significantly prolonged in patients with acromegaly compared to the control group. Furthermore, Tp-e interval, Tp-e/QT ratio, and Tp-e/QTc ratio showed a significant correlation with plasma GH levels and LVMI values. Conclusions: Our study revealed that Tp-e interval, Tp-e/QT ratio, and Tp-e/QTc ratio are prolonged in patients with acromegaly. We believe that the Tp-e interval, Tp-e/QT ratio, and Tpe/QTc ratio can be used in the evaluation of increased cardiovascular risk in patients with acromegaly.

Objectives: Diabetic nephropathy (DN) is a major microvascular complication of diabetes and a leading cause of end-stage renal disease. Reliable biomarkers for early DN detection remain limited. Moesin and podocalyxin, involved in endothelial and podocyte integrity, may contribute to DN pathophysiology. This study evaluated whether serum moesin and podocalyxin levels differ according to the presence of early-stage DN in type 2 diabetes. Methods: In this single-center cross-sectional study, 63 patients with type 2 diabetes and 27 healthy controls were included. Diabetic participants were classified as normoalbuminuric diabetes (DM) or microalbuminuric early DN (DNP) based on spot urine albumin-to-creatinine ratio. Serum moesin and podocalyxin levels were measured by ELISA. Group comparisons and regression analyses were performed. Results: Both biomarkers were significantly lower in diabetic patients than in controls (p<0.001), but no difference was observed between DM and DNP after age adjustment. Neither biomarker independently predicted nephropathy. A composite factor including moesin, podocalyxin, age, white blood cell count, and microalbuminuria was associated with DN (OR 4.03, 95 % CI 1.42–11.47, p=0.009). Conclusions: Serum moesin and podocalyxin decrease in type 2 diabetes but do not independently distinguish early DN, suggesting limited kidney-specific utility.

Gaucher disease (GD) is the most common lysosomal storage disorder caused by biallelic pathogenic variants in GBA1, resulting in deficient β-glucocerebrosidase activity. Clinical characterization of rare GBA1 variants is crucial for understanding disease heterogeneity. We report an extremely rare GBA1 variant previously catalogued at the sequence level in HGMD and UniProtKB, and recently listed in ClinVar without a clinical significance assertion, but lacking phenotypic data. A 23-year-old male presented with fatigue, chills, shivering, and diffuse bone pain. Physical examination revealed hepatosplenomegaly, and laboratory tests showed moderate thrombocytopenia. Leukocyte β-glucocerebrosidase activity was markedly reduced (0.90 nmol/mg/h), and plasma Lyso-Gb1 was highly elevated (431.3 ng/mL; reference T (p.Thr82Ile; rs1141811) variant. Family screening showed heterozygosity in both parents and one sibling. This report provides the first comprehensive clinical characterization of the rare GBA1 p.Thr82Ile variant, expanding the phenotypic spectrum of Type 1 GD.
