Gait impairment in Parkinson's disease (PD) patients is characterized by the inability to generate appropriate stride length. Treadmill training has been proposed as a therapeutic tool for PD patients. However, it remains unknown whether treadmill training effects are different from overground walking training. Thus, our goal was to explore the effects of two training programs, walking on a treadmill and walking overground, in PD patients. 22 PD patients were randomly assigned to a treadmill or overground training group. The training program consisted of 5 weeks (3 sessions/week). Before and after the program we evaluated gait kinematics during walking at preferred and maximal speed; Timed Up and Go (TUG); static posturography and knee extensors strength. Gait parameters were reevaluated in the treadmill training group one month after the cessation of the training. Preferred speed walking improved in both groups after the training program. The treadmill training program, but not the overground, led to an improvement in the stride length at the preferred and maximal walking speed in the PD patients. In addition, the treadmill training group showed improvement of the TUG and static posturography tests. The improvement in gait parameters was maintained one month after the cessation of the treadmill training. This study provides evidence of a specific therapeutic effect of treadmill training on Parkinsonian gait and balance. Walking on a treadmill may be used as an easy, effective and accessible way to improve the stride length and balance in PD patients.

Physiological variables such as maximal oxygen uptake (VO2max), velocity at maximal oxygen uptake (vVO2max), running economy (RE) and changes in lactate levels are considered the main factors determining performance in long-distance races. The aim of this review was to present the mathematical models available in the literature to estimate performance in the 5000 m, 10,000 m, half-marathon and marathon events. Eighty-eight articles were identified, selections were made based on the inclusion criteria and the full text of the articles were obtained. The articles were reviewed and categorized according to demographic, anthropometric, exercise physiology and field test variables were also included by athletic specialty. A total of 58 studies were included, from 1983 to the present, distributed in the following categories: 12 in the 5000 m, 13 in the 10,000 m, 12 in the half-marathon and 21 in the marathon. A total of 136 independent variables associated with performance in long-distance races were considered, 43.4% of which pertained to variables derived from the evaluation of aerobic metabolism, 26.5% to variables associated with training load and 20.6% to anthropometric variables, body composition and somatotype components. The most closely associated variables in the prediction models for the half and full marathon specialties were the variables obtained from the laboratory tests (VO2max, vVO2max), training variables (training pace, training load) and anthropometric variables (fat mass, skinfolds). A large gap exists in predicting time in long-distance races, based on field tests. Physiological effort assessments are almost exclusive to shorter specialties (5000 m and 10,000 m). The predictor variables of the half-marathon are mainly anthropometric, but with moderate coefficients of determination. The variables of note in the marathon category are fundamentally those associated with training and those derived from physiological evaluation and anthropometric parameters.

COVID-19 pandemic restrictions might have negatively affected the health-related physical fitness of children and adolescents. The aim of this study was to contrast the body composition and physical fitness data of two independent samples of children and adolescents obtained from an online database (DAFIS project) before (n = 15,287) and during (n = 2101) the first academic year of the COVID-19 pandemic. The results revealed higher values for the body mass index (p = 0.002), waist circumference (p < 0.001), and waist to hip and waist to height ratios (p < 0.001) during than before the pandemic, particularly in the case of boys. On the other hand, lower muscular fitness was observed for girls during the pandemic. Quantitative and qualitative analysis did not detect relevant changes in cardiorespiratory fitness in children or adolescents (p > 0.05). Our data suggested that pandemic constraints might have affected body composition and muscular fitness of children and adolescents. These results might be of interest for designing specific interventions oriented toward counteracting the negative effects of pandemic restrictions on health-related physical fitness.

Paraneoplastic limbic encephalitis is an autoimmune syndrome characterized by the acute or subacute onset of encephalopathy, memory loss, confusion, temporal lobe seizures, and behavioral and mood changes. Although most patients with paraneoplastic limbic encephalitis have antineuronal antibodies, advances in the field now permit the diagnosis without autoantibody test results. In this case illustrating the new diagnostic criteria, we report a 70-year-old woman who was brought to the emergency room after the acute onset of cognitive impairment, altered mental status, and choreoathetoid movements. Brain magnetic resonance imaging showed hyperintense signals in both temporal lobes, and a chest computed tomogram revealed a thymoma. Because the patient met current diagnostic criteria for autoimmune limbic encephalitis, we were able to start treatment before her antibody tests were processed. The patient received immunotherapy and her tumor was resected. Her choreoathetoid movements disappeared and her other neurologic symptoms improved. Her cerebrospinal fluid proved to be negative for paraneoplastic limbic encephalitis antibodies. Most but not all patients with paraneoplastic limbic encephalitis associated with thymoma have evidence of paraneoplastic antibodies. Prompt management of the underlying malignancy determines whether patients survive and may minimize future cognitive and functional impairment. Practicing neurologists and psychiatrists should be aware of this diagnosis.
