Serratia marcescens is a Gram-negative opportunistic pathogen renowned for its extensive ecological versatility and clinical significance. Once considered a benign saprophyte, it has now been recognized as a notable etiological agent in nosocomial infections, predominantly affecting immunocompromised hosts. Its pathogenicity is mediated by an array of virulence determinants, including hemolysins, proteolytic enzymes, siderophores, and the biosynthesis of the pigmented secondary metabolite prodigiosin, which exhibits notable anticancer and immunomodulatory activities. S. marcescens exhibits proficient biofilm-forming capabilities that underpin persistent device colonization and confer resilience against antimicrobial therapies. Beyond its clinical impact, S. marcescens is of interest in industrial biotechnology and environmental bioremediation applications. This comprehensive review delineates current insights into its taxonomy, virulence pathways, antimicrobial resistance mechanisms, and emerging biotechnological utilities, emphasizing the dual challenges and opportunities it presents to microbiology and therapeutic development.

Abnormalities of the umbilical cord, related to morphology, placental insertion, number of vessels and primary tumors, can influence the perinatal outcome and may be associated with other fetal anomalies and aneuploidies. The chapter investigates the most important congenital anomalies of this structure. Single umbilical artery appears to be associated with ventricular septal defects and conotruncal anomalies, hydronephrosis, dysplastic kidneys, esophageal atresia, spina bifida, holoprosencephaly, diaphragmatic hernia, and cystic hygromas. Velamentous insertion of the cord can be associated with trisomy 21, spina bifida, ventricular septal defects, and esophageal atresia. A hypoplastic umbilical artery has an artery-to-artery diameter difference of more than 50%; described anomalies include trisomy 21, polyhydramnios, congenital heart disease, and fetal growth restriction. Pseudocysts are more common than true cysts, and they are strongly associated with chromosomal defects and other congenital anomalies, especially omphalocele, hydrops, and trisomy 18. Other benign masses are teratomas, angiomyxomas, and patent urachus. Alterations in morphology and ultrastructure of the umbilical cord should extend the investigation, since there are associations with chromosomal anomalies.

Background: Despite advances in surgical techniques and perioperative management, reliable intraoperative predictors of adverse postoperative outcomes in cardiac surgery remain elusive. This study aimed to identify perioperative factors associated with prolonged intensive care unit (ICU) stay and in-hospital mortality while defining actionable thresholds. Methods: A retrospective analysis was conducted on 130 adult cardiac surgery patients (with a median age of 61 years, 66.2% men) from October 2022 to November 2024. Data on preoperative risk factors, intraoperative variables (cardiopulmonary bypass time-CPBT, aortic cross-clamp time-AXCT), and postoperative outcomes (ICU length of stay, in-hospital mortality) were extracted from electronic medical records. Results: Prolonged ICU stay (≥7 days) occurred in 38.5% of patients, and in-hospital mortality was 10%. AXCT was the sole independent predictor of prolonged ICU stay (OR = 1.046, 95% CI = 1.014–1.080, p = 0.005), with a 110-min cut-off (sensitivity 71%, specificity 61%, AUC = 0.729). A Kaplan–Meier analysis showed significantly longer ICU stays above this threshold (p = 0.006). For in-hospital mortality, prolonged CPBT (OR = 1.030, 95% CI = 1.003–1.057, p = 0.030), emergency surgery (OR = 0.043, 95% CI = 0.002–0.863, p = 0.040), and higher AXCT (OR = 0.965, 95% CI = 0.934–0.997, p = 0.034) were the independent predictors. A receiver operating characteristic analysis identified 140 min for AXCT (sensitivity 67%, specificity 70%, AUC = 0.707) and 227 min for CPBT (sensitivity 83%, specificity 69%, AUC = 0.824) as the optimal cut-offs. A combined model (emergency surgery yes/no, AXCT > 140 min, CPBT > 227 min) yielded excellent discrimination (AUC = 0.846). Conclusions: These findings suggest perioperative benchmarks that may guide surgical teams in refining operative strategies, reducing ICU resource utilization, and improving survival following cardiac surgery.

Preeclampsia involves an angiogenic imbalance, but circulating vascular endothelial growth factor A (VEGF A) remains inconsistently described, particularly in relation to maternal adiposity. We studied 90 second-trimester pregnancies, 30 uncomplicated and 60 with preeclampsia, recording maternal body mass index (BMI) and gestational age at sampling. Serum soluble fms-like tyrosine kinase 1 (sFlt1), placental growth factor (PlGF), and VEGF A were measured by enzyme-linked immunosorbent assay (ELISA), and the sFlt1-to-PlGF ratio was calculated. Preeclampsia was associated with higher pre-pregnancy and pregnancy BMI, lower PlGF, and an approximately threefold higher sFlt1-to-PlGF ratio, while sFlt1 alone was only borderline higher. VEGF A was elevated in preeclampsia and rose across higher sFlt1-to-PlGF ratio categories, supporting the interpretation of VEGF A within the integrated sFlt1,PlGF axis rather than as an isolated signal.

Vulvar cancer is one of the rarest gynecological malignancies. The development of this condition can be associated with either dysplasia linked to human papillomavirus (HPV), primarily affecting younger women, or vulvar dermatoses such as lichen sclerosus, which predominantly affect older women. Over the last decade, the incidence of vulvar cancer has risen by 0.6% annually, while the relative survival rate has declined. Although metastasis to the vulva is uncommon, it can occur, particularly from cancers in nearby organs such as the cervix, bladder, rectum, or anus. More rarely, metastases from breast cancer and renal cell carcinoma have been reported in the vulva. Vaginal metastases from clear cell renal carcinoma are especially rare. In this article, we present the case of a 56-year-old patient diagnosed with clear cell renal carcinoma, who came to our clinic with a lesion on the right labia, which was identified as a metastasis originating from the kidney. Given the rarity of genital metastases in renal cancer, such cases should be examined and discussed to encourage further research and studies.
