Endovascular treatment (EVT) is the treatment method for acute ischemic stroke (AIS) caused by large vessel occlusion. Contrast-induced acute kidney injury (CI-AKI) is a relatively common complication. In this study, we aimed to investigate the incidence and possible risk factors of CI-AKI including systemic inflammation response index (SIRI) and systemic immune inflammation index (SII) in patients undergoing mechanical thrombectomy. We retrospectively analyzed patients who underwent EVT for acute cerebral large vessel occlusion between January 1, 2021, and March 31, 2024. Before the intervention, blood was drawn from the antecubital vein and collected in the appropriate tubes. Serum levels of neutrophils, lymphocytes, platelets, hemoglobin, CRP, and albumin were measured, alongside lipid profiles and liver and kidney function parameters. An increase in serum creatinine of 0.5 mg/dL or more than 25% in the measurements 48 to 72 hours after EVT compared to before was defined as CI-AKI. SII was calculated with the formula of: peripheral (platelet count × neutrophil count)/ lymphocyte count; SIRI was calculated with the formula of: (neutrophil count × monocyte count)/ lymphocyte count. Their predictive value for CI-AKI occurrence was compared. CI-AKI was detected in 54 (19%) of the 281 patients included in the study. In terms of clinical parameters, baseline NIHSS, postthrombectomy, cerebral edema, hemorrhagic transformation, recanalization time, discharge mRS and contrast amount were significantly higher in patients with CI-AKI. SII exhibited the highest discriminative performance in predicting CI-AKI in patients undergoing mechanical thrombectomy, with a threshold of > 1784 (AUC = 0.710, 95% CI: 0.631–0.788, P < .001, sensitivity 42.6%, specificity 89.0%) followed by SIRI (AUC = 0.652, 95% CI: 0.576–0.728, P < .001, sensitivity 88.9%, specificity 37.9%). SII and SIRI were found to be significantly higher in patients with CI-AKI. These scores may be new promising low-grade inflammatory indicators for predicting CI-AKI development.

Background/Objectives: The Naples prognostic score (NPS), reflecting inflammation and nutritional status, has prognostic value, especially in cancer. This study evaluated its ability to predict in-hospital mortality in acute ischemic stroke (AIS) patients undergoing endovascular treatment (EVT). Methods: We retrospectively studied 244 patients with AIS who were admitted between April 2020 and December 2023. Patients were included if they presented within 6 h of symptom onset with evidence of intracranial proximal arterial occlusion. The EVT was performed using aspiration catheters, stent retrievers, or both. The NPS was calculated based on the neutrophil–lymphocyte ratio, lymphocyte–monocyte ratio, and albumin and total cholesterol levels. Results: We found a significant association between higher NPS scores and in-hospital mortality. Patients with a high NPS (3 or 4) had a mortality rate of 41.6% compared to 21.0% in the low-NPS group (0, 1, or 2). The full model incorporating NPS showed superior predictive ability for in-hospital mortality compared with the baseline model (areas under the curve 0.881 vs. 0.808). A receiver-operating characteristic analysis at a cutoff of >2.5 for the NPS showed a sensitivity of 86.6% and specificity of 41.9%. This study demonstrated that incorporating the NPS into the predictive model improved the accuracy and calibration for predicting in-hospital mortality. A decision curve analysis showed the net benefit of using the full model incorporating NPS over the baseline model, emphasizing its potential clinical application in prognostication. Conclusions: NPS is a reliable predictor of in-hospital mortality in AIS patients undergoing EVT. Incorporating NPS into clinical practice could help to identify high-risk patients and improve outcomes through tailored interventions.

Background: Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system characterized by focal demyelinating lesions, axonal dysfunction/degeneration, and gliosis, which can lead to various clinical disabilities. Visual evoked potentials (VEP) is sensitive and repeatable techniques capable of monitoring significant short-term changes in neuroaxonal integrity and alterations in nerve conduction triggered by acute optic neuritis. Aim: This study aims to evaluate whether VEP latency and amplitude could serve as subclinical predictors of clinical outcomes in MS patients over short-term follow-ups. Methods: This study was planned to include MS patients diagnosed according to the McDonald Criteria 18 who did not have any psychiatric, neurological, or ocular disorders that could interfere with the main purpose. The VEP test was performed for routine evaluation of demyelination or axonal damage. Results: A total of 83 patients were included in the study, with a mean age of 33.6 ± 9.3 years. Of all the patients, 54 were female (65.1%) and 29 were male (34.9%). Right pattern reversal visual evoked potential (PVEP) P100 (OR for PVEP1: 0.802, p = 0.001; OR for PVEP2: 0.879, p = 0.002) was statistically significant in showing right VEP abnormality at both baseline and at 6 months. Left VEP abnormalities were associated with left PVEP P100 at PVEP1 (OR: 0.852, p = 0.003) and left PVEP N75 at PVEP2 (OR: 0.935, p = 0.029). Conclusion: VEPs have the potential to predict short-term subclinical stability or progression, making them valuable candidates for early treatment adjustments and evaluating future pharmacotherapy-supported remyelination.

Background and purpose - Fibrinogen to albumin ratio (FAR) is thought to have a predictive effect in diseases such as cancer and myocardial infarction. We aimed to eluci date the prognostic value of FAR in ischemic stroke patients who underwent mechanical thrombectomy. Methods - A total of 103 patients hospita lized for acute stroke who underwent me cha nical thrombectomy within 6 hours of symp toms' outset have been analyzed retro spectively. Stroke severity was interpreted via the National Institutes of Health Stroke Scale (NIHSS) score during the neurological examination. Recanalization success after mechanical thrombectomy was evaluated with the TICI score (Thrombolysis in Cerebral Infarction scale), and 2b - 3 patients were recorded as those with recanalization. The patients' modified Rankin scale (mRS) at discharge and at the end of the third month were recorded. Results - Statistically significant differen ces were observed in age, admission blood glucose, glomerular filtration rate and FAR according to the mRS scores of the patients in the third month (p<0.05). Significant va riab les in the risk factor analysis were reevaluated in the multivariate model. The best model was determined using the back ward Wald method in the multivariate model, and it was determined that differences in age, admission blood glucose, and FAR were significant. Conclusion - FAR can be used as a novel, effective, economical, and practical biomark er in patient with acute ischemic stroke who underwent mechanical thrombectomy.

This study aimed to evaluate the presence and comparison of microvascular abnormalities in essential tremor (ET) and Parkinson disease (PD) using optical coherence tomography angiography (OCT-A) and to show the relationship between retinal microvascular changes and disease stage in the patient group. A total of 176 eyes, including 26 PD diagnosed according to the United Kingdom PD Society Brain Bank criteria, 31 ET diagnosed according to the Washington heights-inwood genetic study of ET (WHIGET) criteria and 31 healthy controls, were included in the study. Unified PD assessment scale (UPDRS) motor scores, non-motor symptom scale (NMS), modified Hoehn&Yahr stages (mH&Y) and Fahn–Toloso–Marin grading scale were recorded. All patients were evaluated in terms of visual acuity, fundus examination, intraocular pressure measurements, and refractive errors in Ophthalmology department. Deep macular vascular density was significantly decreased in PD compared to both the controls and ET(P < .05). In the measurements in the inferior quadrant of the retinal nerve fiber layer (RNFL) of the optic disc (OD), the values of the controls were significantly higher than those of PD in both eyes (P = .014 and P = .010). Radial peripapillary capillarity density in the left eye was substantially lower in ET than in controls (P = .045). In both eyes, OD radial peripapillary capillarity inside the disc small values of PD were significantly lower than those of ET and controls (P < .05). In our study, deep macular vascular density, RNFL and radial peripapillary capillarity were significantly lower in PD compared with ET and control groups, and radial peripapillary capillarity was lower in ET compared with control group. This study provides valuable information regarding the potential of OCT-A as a diagnostic tool for PD and ET.
