Background/Objectives: The most prevalent benign tumors in women are uterine fibroids. Most patients—more than half—do not exhibit any symptoms, but the most common clinical signs include irregular uterine bleeding, pelvic pain, gastrointestinal problems, increased frequency of urination, and, in some cases, infertility. Venous thromboembolism is a very rare consequence, especially when significant uterine fibroids are present. This syndrome usually develops because of pelvic vascular systems being compressed, which causes venous stasis. Pharmacological treatment, minimally invasive procedures, and surgical techniques are examples of therapy alternatives. The purpose of this study is to present, compare, and potentially elucidate the underlying mechanisms of VTE development in fibroids. Methods: we have synthesized findings from 24 documented instances of venous thromboembolism (VTE) linked to uterine fibroids. Results: the principal mechanism underlying thromboembolic events was identified as the mechanical compression of pelvic venous structures due to mass effect. Additionally, we recognized other pertinent risk factors, including oral contraceptive use, May-Thurner syndrome, myomatous erythrocytosis, and intravenous leiomyomatosis. None of the reviewed case reports provided evidence of confirmed inherited thrombophilia in the patients under investigation. The femoral and popliteal veins, primarily in the left leg, were most frequently impacted by thrombosis and the ensuing blockage. Imaging techniques confirmed that individuals suffered pulmonary embolisms in half of the cases. When the right treatment was given as soon as possible, most of VTEs had favorable outcome. In almost half of the cases examined, the patient had a hysterectomy. Since all symptoms were alleviated and the chance of additional thromboembolic consequences was reduced, this treatment strategy turned out to be the most successful. Conclusions: Clinicians should maintain a low threshold for venous imaging in women with large pelvic masses and unilateral limb symptoms. Despite being uncommon, VTE associated with UFs can cause serious morbidity. Mechanical venous compression is the main mechanism, which is often exacerbated by additional prothrombotic variables. Clinicians should maintain a low threshold for venous imaging in women with significant pelvic masses and unilateral limb symptoms, look for concurrent thrombophilia, and investigate early surgical consultation to address compressive etiologies when VTE is still unexplained. It would be simpler to ascertain the actual incidence and pinpoint risk variables that can be altered with standardized reporting of fibroid-associated VTE and prospective registries.

Introduction. Li-Fraumeni syndrome (LFS) is a hereditary familial predisposition to a wide range of certain, often rare malignant diseases. Patients also have an increased risk of developing secondary and even tertiary malignancies throughout their lifetime. The most common malignancies are soft-tissue and bone sarcomas, breast cancer, brain tumors, adrenocortical carcinoma, and acute leukemia. The syndrome is inherited as an autosomal dominant disorder. In most families with LFS, germline mutations of the tumor protein have been identified on the TP53 gene. To our knowledge, this is the second case report of LFS that has been reported in our country so far. Case report. We present five members of the same family with malignant diseases typical for LFS. A woman at the age of 21 had recurrent astrocytoma and mediastinal liposarcoma. Her older sister had rhabdomyosarcoma and liver cancer and died at the age of 18. The mother of their father was diagnosed with breast cancer at the age of 45, and she died at the age of 52. The father?s sister had osteosarcoma and died before the age of 40. The father was diagnosed with lung adenocarcinoma at the age of 49, two years after the death of his second daughter. Genetic analysis identified a pathogenic, heterozygous germline mutation of the TP53 gene. He also has a third, 8-year-old daughter for whom he denied testing for LFS. Conclusion. Genetic analysis for LFS of all family members is required in patients with rare and multiple malignancies but also frequent and early onset malignancies in the family. Screening for the detection of early cancer manifestation is the key to prolonged survival in people with LFS.

Introduction. Tuberculosis (TB) continues to be a global health problem, with various manifestations affecting different organs and organ systems. Tuberculous lymphadenitis (TBL), also known as scrofula or lymph node (LN) TB, is a manifestation of Mycobacterium tuberculosis infection primarily affecting the LNs. Our region has a low annual incidence rate of TB, which is why the expected prevalence of TBL is also low. Case report. We present two cases of TBL diagnosed three months apart. Both presented patients had the disease localized in the LNs of the neck. These were young people without chronic diseases and previously in good health condition. The diagnosis was confirmed by surgical biopsy of the LNs in the first patient and fine needle aspiration biopsy in the second patient. Both patients were treated with standard protocols for TBL, with favorable outcomes and without significant complications and side effects. Conclusion. Presented cases highlight the importance of considering TBL in the differential diagnosis of neck masses, even in regions with a low prevalence of TB. Timely diagnosis and treatment are essential for preventing complications and ensuring a successful outcome.

Background and Objectives: A number of published studies comparing transanal haemorrhoidal dearterialization (THD) and conventional haemorrhoidectomy (CH) have primarily focused on short-term outcomes, while data on long-term results remains scarce. This study aimed to compare Doppler-guided THD and CH with emphasis on both early and long-term postoperative results. Methods: This retrospective study included 336 patients who underwent CH (Group 1, n = 189) or Doppler-guided THD (Group 2, n = 147). The groups were compared according to demographic data, HD stage, symptoms, complications, duration of hospital stay, postoperative pain, patient satisfaction, and recurrence rates after 18 months of surgery. Results: There was no difference between groups regarding gender, although there were statistically significant distinctions in terms of disease stage and age (p < 0.001), with the emphasis that in CH group patients had advanced stage of disease and were older. THD was associated with significantly lower postoperative pain, with median visual analogue scale (VAS) scores on postoperative days 1, 2, and 7 of 5, 3, and 1, compared with 7, 5, and 3 after CH (p < 0.001). There was no difference in VAS on POD 30 between groups. Median hospital stay was significantly shorter after THD (1 vs. 3 days; p < 0.001). Patients undergoing CH required more analgesics, laxatives, and follow-up visits within the first postoperative month (p ≤ 0.008). Postoperative complications occurred in nine patients, without significant differences between groups. During the 18-month follow-up period, HD recurrence occurred in seven patients and was more frequent after THD. Additionally, two THD patients developed anal fissures. Conclusions: Doppler-guided THD is a safe and effective treatment for HD, providing reduced postoperative pain, faster recovery, and shorter hospitalization. Long-term outcomes were favourable in both groups, with a trend of lower recurrence rate in the CH group.

Introduction. Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy caused by structural and functional abnormalities of primary cilia, leading to disruption of key signaling pathways. It is clinically characterized by progressive retinal degeneration, polydactyly, early-onset obesity, cognitive impairment, hypogonadism, and renal abnormalities. Malignant mesothelioma (MM) is a malignant tumor of mesothelial cells that most commonly affects the pleura and is strongly associated with asbestos exposure and poor prognosis. In addition to environmental factors, genetic predisposition may contribute to its pathogenesis. Case report. We present a 27-year-old male with BBS who was evaluated for chest pain and progressive dyspnea. Pleural fluid analysis and video-assisted thoracoscopic pleural biopsy confirmed epithelioid pleural MM. The patient received systemic chemotherapy with pemetrexed and cisplatin, together with palliative treatment for recurrent pleural effusions. Therapeutic options were further limited by the underlying genetic disorder. Despite advanced disease and limited treatment options, the patient achieved unexpectedly prolonged overall survival of 7 years from the time of MM diagnosis. Conclusion. This case illustrates the diagnostic and therapeutic complexity of managing malignancies in patients with rare genetic syndromes and emphasizes the importance of an individualized multidisciplinary approach. It also raises the possibility that ciliary dysfunction may contribute to mesothelioma pathogenesis, warranting further molecular and clinical investigation.
