Introduction: Peripheral artery disease (PAD) is a common manifestation of atherosclerosis. Despite the effectiveness of endovascular therapy (EVT), patients with PAD often face poor prognoses. This study investigates the relationship between specific inflammatory indices and wound healing in patients with Fontaine stage 4 chronic limb-threatening ischemia (CLTI). Methods: From June 2021 to January 2024, 168 patients with Fontaine stage 4 CLTI, totaling 185 affected extremities, who underwent successful EVT, were assessed retrospectively. Patients were categorized based on wound healing post-EVT. The Naples prognostic score (NPS), pan-immune inflammation value (PIV), systemic immune-inflammation index (SII), and systemic inflammation response index (SIRI) were calculated. Discrimination and decision curve analyses were used to explore the link between inflammation and wound healing. Results: Wound healing was observed in 142 (76.8%) patients post-EVT. Nonhealing patients exhibited higher indices of NPS, PIV, SII, and SIRI. NPS (aOR = 0.381, 95% CI 0.215–0.675, p = 0.001), PIV (aOR = 0.997, 95% CI 0.996–0.999, p < 0.001), SII (aOR = 0.997, 95% CI 0.996–0.997, p < 0.001), and SIRI (aOR = 0.443, 95% CI 0.313–0.625, p < 0.001) were independently predictive of wound healing. SIRI (AUC = 0.840, 95% CI 0.777–0.904) demonstrated superior predictive ability compared to PIV (AUC = 0.799, 95% CI 0.722–0.876, p dif < 0.001), SII (AUC = 0.788, 95% CI 0.712–0.865, p dif < 0.001), and NPS (AUC = 0.760, 95% CI 0.681–0.838, p dif < 0.001). SIRI also showed higher net reclassification improvement over PIV (68.4%, p dif < 0.001), SII (38.5%, p dif = 0.024), and NPS (29.8%, p dif = 0.079). All inflammatory indices, especially SIRI, provided prognostic value in determining wound healing at high treatment thresholds (> 40%). Conclusion: In patients with CLTI undergoing EVT, SIRI, SII, PIV, and NPS may help predict the potential for wound healing during in-hospital follow up, with SIRI being the strongest predictor.

Background: To investigate the relationship between anxiety and white coat hypertension (WCH) using the hospital anxiety and depression scale-anxiety (HADS-A) score. Methods: Participants lacking a pre-existing diagnosis of hypertension but displaying increased office blood pressure were included in this study. Subsequently, they were classified as either newly diagnosed sustained hypertension (SustHT) or white coat hypertension (WCH) patients, as determined by 24-hour ambulatory blood pressure monitoring measurements. The assessment of their anxiety levels was conducted using the HADS-A questionnaire. We performed regression, comparative, and sensitivity analyses to elucidate the association between anxiety and WCH. Results: In this cohort of 303 consecutive individuals (mean age 54 years, 46% female), 81 (26.7%) patients were diagnosed with WCH. Those with WCH were younger (49 vs. 56 years, P < .001), had higher heart rate (85 vs. 76 bpm, P < .001) and exhibited a female predominance (56% vs. 43%, P = .049) compared to individuals with SustHT. The HADS-A was higher in WCH than in SustHT (9.0 ± 2.9 vs. 6.6 ± 2.6, P < .001). Furthermore, HADS-A showed positive correlation with systolic and diastolic pressures measured in the out-patient clinic (r = 0.523 and r = 0.387, respectively; P < .001 for both). The full model with HADS-A had better discriminatory power (Harrell’s c-index 0.82 vs. 0.77, P = .0025), increased calibration, and a greater net benefit than the base model without. The ROC curve analysis, using a cut-off of >6 for HADS-A, demonstrated a sensitivity of 76.5% and specificity of 53.6% in detecting WCH (Area Under the Curve = 0.72, P < .001). Conclusions: Our study revealed that individuals with WCH, in comparison to those with SustHT, exhibit a higher level of anxiety as indicated by HADS-A.

Objectives: Infective endocarditis (IE) remains a significant clinical challenge due to its high morbidity and mortality rates. Prognosis is influenced by patient characteristics, causative microorganisms, complications, and echocardiographic findings. The Barcelona Bio-Heart Failure (BCN Bio-HF) Risk Score, widely used in heart failure, has not yet been evaluated in IE. This study aimed to assess the association between the BCN score and in-hospital mortality in patients with IE. Methods: This retrospective, single-center observational study included 108 patients diagnosed with IE. Patients were divided into two groups based on the occurrence of in-hospital mortality, which was defined as any death occurring during hospitalization. Clinical, demographic, laboratory, and echocardiographic data were compared between the groups. Results: Among 108 patients, 29 (26.9%) experienced in-hospital mortality. Compared to survivors, non-survivors were older (p=0.046) and had a higher prevalence of chronic kidney disease (p=0.041). Staphylococcus aureus and methicillin-resistant strains were more common in the mortality group (p=0.043 and p=0.04, respectively). Echocardiographic findings showed a lower ejection fraction (p=0.03), higher pulmonary artery systolic pressure (p=0.017), and larger vegetation size (p=0.033) in non-survivors. Mechanical complications were also more frequent (p=0.017). Laboratory results revealed lower hemoglobin (p=0.012) and higher levels of WBC (p=0.035), CRP (p=0.01), procalcitonin (p=0.02), NT-proBNP (p=0.008), and BCN Bio-HF risk score (p=0.008) in patients with in-hospital mortality. Conclusion: The BCN Bio-HF risk score showed potential in predicting in-hospital mortality in infective endocarditis by integrating clinical and biomarker data. Future studies should aim to develop IE-specific prognostic models incorporating dynamic clinical variables and novel biomarkers to enhance risk stratification and guide management.

Background: To investigate the association between the uric acid-to-high-density lipoprotein cholesterol ratio (UAHDLr) and the risk of new-onset atrial fibrillation (NOAF) during hospitalization in patients with non-ST-elevation myocardial infarction (NSTEMI). Methods: This retrospective cohort study included NSTEMI patients without prior atrial fibrillation. UAHDLr was log2-transformed to evaluate the effect of doubling. Feature selection was performed using least absolute shrinkage and selection operator regression. Cox proportional hazards models with sequential adjustments were applied. Nonlinear associations were assessed using restricted cubic spline analysis, and discrimination was evaluated with time-dependent receiver operating characteristic analysis. Results: UAHDLr was independently associated with in-hospital NOAF across all models. Doubling (1-unit increase) UAHDLr significantly increased NOAF risk in unadjusted (HR: 5.97, 95% CI: 3.71–9.61) and clinically adjusted models (HR: 5.98, 95% CI: 3.58–9.99). The association was stronger in the LASSO-adjusted (HR: 8.19, 95% CI: 4.13–16.24) and fully adjusted models (HR: 13.40, 95% CI: 5.90–30.44). Spline analysis showed a progressive increase in NOAF risk with higher UAHDLr values. Discrimination was stable, with an area under the curve of approximately 0.76. Conclusions: UAHDLr is a strong, easily accessible biomarker for predicting in-hospital NOAF in NSTEMI patients and may support early risk stratification.
