This study aimed to explore the potential association between the triglyceride–glucose index (TyG) and the atherogenic index of plasma (AIP)—both considered surrogate markers for atherosclerosis—and major adverse cardiovascular events (MACEs) in patients diagnosed with chronic coronary syndrome (CCS). We conducted a retrospective analysis, encompassing 715 consecutive patients with intermediate CCS risk, who presented at the outpatient clinic between June 2020 and August 2022. MACEs included non-fatal myocardial infarction, hospitalization for heart failure, cerebrovascular events, non-cardiac mortality, and cardiac mortality. The primary outcome was the composite occurrence of MACEs during the follow-up period. For time-to-event analysis of the primary outcome, we employed Kaplan–Meier plots and Cox proportional hazard models. The median age of the overall study population was 55 years, with a median follow-up duration of 17 months. Multivariate Cox regression analysis identified age, hypertension, Coronary Artery Disease–Reporting and Data System score, and TyG index as independent predictors of the primary outcome. Notably, individuals with high TyG levels exhibited a significantly higher primary outcome rate compared to those with low TyG levels (18.7% vs. 3.8%, p < 0.001). Similarly, patients with elevated TyG values demonstrated statistically higher rates of cerebrovascular events, hospitalizations for heart failure, non-fatal myocardial infarctions, non-cardiac mortality, and cardiac mortality. These findings suggest that TyG may serve as a predictive marker for adverse cardiovascular outcomes in patients with CCS.

Infective endocarditis complicated by a coronary sinus abscess is extremely rare and can lead to conduction issues. We report a 63-year-old hemodialysis patient with Methicillin-resistant Staphylococcus aureus infective endocarditis who developed complete atrioventricular block. Transesophageal echocardiography and cardiac computed tomography showed vegetations on the posterior mitral annulus, severe mitral regurgitation, and a coronary sinus abscess. Atrioventricular conduction recovered spontaneously, probably due to abscess drainage into the left atrium. Despite surgery, the patient died from cardiogenic shock. This case highlights the diagnostic and therapeutic challenges of periannular complications.

Acute ST-elevation myocardial infarction with coronary artery ectasia is rare and difficult to treat. The complication rate is high in percutaneous intervention. At the same time, a standard treatment cannot be performed due to the lack of sufficient studies. In this case, complex complications and a different solution technique are presented. Here, we present our experience with a case of acute inferior myocardial infarction in a 62-year-old man with an ectatic right coronary artery and a 1-year follow-up report.

This section will address the main imaging methods used to evaluate coronary collaterals in patients with chronic total occlusion (CTO), including coronary angiography, intravascular ultrasound (IVUS), cardiac magnetic resonance imaging (MRI), coronary computed tomography angiography (CTA), and optical coherence tomography (OCT). The role of each method in imaging coronary collaterals in CTO patients, along with their advantages, disadvantages, and relative superiority to one another, will be discussed. Each method has its own unique advantages and disadvantages in assessing collateral circulation in patients with CTO, and the optimal imaging modality should be determined based on the clinical situation, available resources, and patient characteristics. A comprehensive evaluation of collateral circulation is crucial for determining the best treatment strategy in patients with CTO.
