Objective: Coronavirus 2019 disease presents in a spectrum that can range from mild viral infection to pneu- monia. Common symptoms of coronavirus disease 2019 pneumonia include cough, sputum, and shortness of breath. High-frequency chest wall oscillation is a pulmonary rehabilitation method used for the recovery of pulmonary functions and removal of secretions in the lungs. The aim of the study was to evaluate the efficacy of high-frequency chest wall oscillation on patients with coronavirus disease 2019 pneumonia. Materials and Methods: In this study, 100 patients, between 18 and 70 years old, with a positive polymerase chain reaction result for coronavirus disease 2019, were included. Standard medical treatment was applied to all patients. In group rehabilitation, high-frequency chest wall oscillation treatment was applied twice a day for 20 minutes for 5 days. No additional intervention was made to the control group. Pulmonary function tests and oxygenation were evaluated on the first and fifth days. Patients’ high-flow oxygen, non-invasive mechani- cal ventilation, and invasive mechanical ventilation needs were evaluated and recorded. Results: Compared with the control group, the forced expiratory volume in 1 second, forced vital capacity, and peak expiratory flow rates were statistically higher in the rehabilitation group on the fifth day (P < .05). On evaluating the oxygenation of patients, the fifth day to first-day oxygen saturation difference was signifi- cantly higher in rehabilitation group than in control group (P < .05). Furthermore, the number of patients who needed non-invasive mechanical ventilation was lower in the rehabilitation group (P < .05). Conclusion: This study demonstrated that pulmonary rehabilitation applied with the high-frequency chest wall oscillation device in patients with coronavirus disease 2019 in the early period contributed to the improvement of oxygenation by providing significant improvement as observed in the pulmonary function tests of the patients. Background/Objectives: Shoulder arthroscopies are commonly conducted in orthopedic practice. The interscalene brachial plexus block (ISB) is regarded as the “gold standard” for postoperative analgesia in shoulder surgeries. The serratus posterior superior intercostal plane block (SPSIPB) was introduced as an innovative treatment for addressing thoracic and shoulder discomfort. This study aims to examine the effects of SPSIPB and ISB techniques on postoperative pain levels, opioid intake, and respiratory function measures in patients having shoulder arthroscopy. Methods: Patients were divided into two groups. In the ISB group, 15 mL of fluid containing 0.25% bupivacaine was applied between interscalene muscles, while in the SPSIPB group, 30 mL of 0.25% bupivacaine was applied in the fascial plane between the serratus posterior superior muscle and the intercostal muscles. Results: There were no statistically significant differences in demographic characteristics (p > 0.05). VAS scores were statistically lower in the ISB group compared to the SPSIPB group at rest at 1, 2, 4, 8, 12, and 24 h postoperatively in the PACU (p < 0.05). VAS scores were also lower in the ISB group compared to the SPSIPB group during active movement at 1, 2, 4, 8, and 12 h postoperatively in the PACU (p < 0.05). Twenty-four-hour fentanyl consumption was lower in the ISB group compared to the SPSIPB group (407.50 ± 169.32 μg and 767.50 ± 178.00 μg, respectively, p < 0.001). The decrease in FEV1 and FVC was higher in the ISB group compared to the SPSIPB group (p < 0.001). Conclusions: ISB effectively relieves pain during shoulder arthroscopic procedures; however, while SPSIPB is considered a more advantageous option in terms of respiratory safety, it may not provide adequate analgesia on its own.

Aims: The primary aim of this study was to investigate the effects of two different low-dose bupivacaine used in cesarean operations on intraoperative hemodynamic parameters, and the secondary aim was to evaluate the time of anesthesia onset, motor block scores, and the need for vasopressor agent. Methods: This prospective randomized double-blind study was performed on 72 pregnant women who underwent cesarean surgery. We randomized the patients into two groups, Group A and Group B, and performed a combined spino-epidural anesthesia. Group A received a solution containing 5 mg isobaric bupivacaine+15µg fentanyl+0.5 isotonic (total volume 1.8 ml) and Group B received a solution containing 7.5 mg bupivacaine+15µg fentanyl (total volume 1.8 ml) over a period of 30 seconds. Vital signs were recorded before the spinal anesthesia and perioperatively. Demographic data, hemodynamic parameters, vital signs and side effects, operation duration, time interval from spinal injection to placement in the supine position, Apgar scores, time interval from spinal injection to delivery, analgesia duration, the degree of motor block immediately before the surgery and at the end of the operation, maximum block level, time for sensory block to reach T6 dermatome level after spinal injection and postoperative side effects were recorded. We recorded the postoperative time to resolution of motor block and the time to regression of sensory block to T10. Results: There was no statistically significant difference between the two groups in terms of demographic data, anesthesia duration, surgery duration, time interval from spinal injection to placement in the supine position, time interval from spinal injection to delivery, analgesia duration, time for sensory block to reach T10, T6, and T4 dermatomes, 1- and 5-minute (min) Apgar scores, and preoperative Bromage scores. While the mean time to resolution of motor block was 159.69±65.72 min in Group B, the mean time to resolution of motor block was 123.13±64.93 min in Group A and the difference was statistically significant (p=0.02). Hypotension was observed in 19 patients (52.77%) in Group A and 29 patients (80.55%) in Group B (p=0.012). A statistically significant difference was detected between the two groups in terms of the need for vasopressor agent and the amount of ephedrine used (p=0.012, p=0.021, respectively). Postoperative Bromage score was 1.25±0.93 in Group A while it was 2.47±1.27 in Group B (p=0.000). Conclusion: In patients undergoing CS, we found that intrathecal administration of 5 mg isobaric bupivacaine combined with 15 mcg fentanyl and 0.5 ml isotonic not only provided adequate anesthesia but also better preserved hemodynamic stability and significantly shortened the time to resolution of motor block. We believe that this dose can be used safely in patients undergoing CS. Further studies using varying intrathecal bupivacaine doses are necessary to validate our findings.

Objective: Central venous catheters are mainly used for safe fluid infusion, total parenteral nutrition, and evaluation of hemodynamic parameters. Complications related to central venous access can occur during insertion, after insertion or during catheter maintenance. The aim of this study was to investigate the demographic data and the presence of catheter-related complications in pediatric patients with central venous catheters in our hospital. Methods: This study retrospectively reviewed demographic data and the presence of catheter-related complications in pediatric patients with central venous catheters in our hospital from June 1, 2019, to February 28, 2023. Results: Patients who underwent central venous catheterisation in our hospital between June 2020 and February 2023 were included in the study. When the demographic data of the patients were evaluated, 141 (57.8%) of our patients were male and 103 (42.2%) were female, and there was no statistically significant difference between the groups. In the femoral group, catheterisation of the right femoral vein was preferred more than the left vein (72.2%-27.7%), whereas in the jugular-subclavian group, the right internal jugular vein was preferred more than other sites. The risk of infection was statistically higher in the femoral group compared to the subclavian group. Conclusion: In clinical practice, we think that the risk of complications can be reduced in central venous catheter applications in pediatric patients by experienced personnel by complying with asepsis/antisepsis conditions and using appropriate imaging devices.

JOURNAL/mgres/04.03/01612956-202606000-00005/figure1/v/2026-04-21T115516Z/r/image-tiff Low-flow anesthesia aims to minimize anesthetic gas consumption while maintaining adequate anesthesia. To examine the effects of minimal-flow anesthesia on perioperative lung dynamics and postoperative pulmonary function tests, a prospective, randomized controlled study was conducted between October 2023 and March 2024 at Atatürk University. A total of 66 patients (15 males, 45 females) with confirmed American Society of Anesthesiologists (ASA) grade I–II, aged 18–65 years, and scheduled for elective laparoscopic cholecystectomy were included in the study. Patients were randomized into two groups: MeFA (medium flow anesthesia, 2 L/min fresh gas flow) and MiFA (minimal flow anesthesia, 0.5 L/min fresh gas flow). In both groups, dynamic compliance values, peak inspiratory pressure (PIP) values, total inhalation anesthetic drug consumption, total remifentanil drug consumption, duration of anesthesia, duration of surgery, and spirometry test results were recorded. Respiratory measurements were recorded at the 5 th minute after intubation (T1), 5 th (T2), 10 th (T3), 30 th (T4), and 60 th (T5) minutes after surgical incision and immediately after the surgical suturing (T6) pulse. There was no significant difference in compliance or PIP values between the groups from T1 to T5 ( P > 0.05). However, at T6, the MeFA group exhibited a significant decrease in compliance and an increase in PIP compared with the MiFA group ( P < 0.05). Additionally, significant differences in compliance and PIP values were found across all time intervals compared with those at T1, except for the T5–6 compliance values in the MiFA group ( P < 0.001). No significant difference in respiratory function test values was noted between the groups ( P > 0.05). The MiFA group exhibited a relatively milder reduction in compliance values and a lesser elevation in PIP values. Compared with medium-flow anesthesia, minimal-flow anesthesia may help mitigate perioperative lung function deterioration. These findings suggest potential benefits in preserving lung mechanics, warranting further research. This trial was registered at clinicaltrials.gov (identifier No. NCT06055335, registered March 25, 2023).
