Bloodstream infections (BSIs) caused by carbapenem-resistant Klebsiella pneumoniae (CRKP) are a serious threat to public health. In this study, it was aimed to evaluate the risk factors, treatment, length of hospitalisation and mortality of patients with BSI caused by CRKP. Between October 2021 and October 2023, patients aged 18 years and older who were treated for BSI at Mersin University Faculty of Medicine Hospital and had pathogens identified as CRKP or carbapenem-sensitive Klebsiella pneumoniae (CSKP) were retrospectively reviewed and included in the study. Of the 107 patients with BSI included in the study, 66 (61.7%) had CRKP and 41 (38.3%) had CSKP. The risk factors associated with CRKP in BSIs were found to be acute renal failure (p = 0.048), mechanical ventilation (MV) (p = 0.042), post-earthquake period (p = 0.022) and use of carbapenem (p < 0.0005) and other beta-lactam antibiotics (p = 0.039). Mortality was significantly higher in BSIs caused by CRKP (80.3%/29.3%, p < 0.0005). In the CRKP group, intensive care unit follow-up (p = 0.020), intubation (p = 0.039), MV monitoring (p = 0.025) and concomitant pneumonia (p = 0.043) significantly increased mortality. There were no significant differences in treatment outcomes between ceftazidime-avibactam and carbapenem combinations (p > 0.05). In the CRKP group, the duration of treatment and hospital stay were significantly longer in patients who started treatment three days or later (p = 0.036/p = 0.011). This study demonstrates that identifying risk factors for BSIs caused by CRKP and implementing early treatment protocols may lead to a reduction in treatment time and mortality rate.

Objectives: In this scientometric and quantitative study, the previous literature on the topic of Cytomegalovirus (CMV) in solid organ transplant (SOT) recipients was analysed. Our overarching goal was to explore the publications in this field and to identify research hotspots. Material and Methods: The Web of Science (WoS) database was utilized to research publications. The following terms were included in the search strategy syntax: "CMV" or "CMV infection" or "Cytomegalovirus" and "solid organ" or "hepatic" or "liver" or "pancreatic" or "lung" or "heart" or "transplantation" or "organ donation". Articles were selected as the document type. The complete text data of the included publications was retrieved from the WoS database. The VOSviewer (version 1.6.10, Leiden University, Netherlands) was used to do the bibliometric study in order to display the collaboration network, emphases, and future trends in the relevant topic. Results: 2432 results were reached from the Wos Core Collection. 1390 of them were articles. The USA was the top-ranked country (29.928%). Germany, Japan, England, and France (6.043%) were ranked in the first 5 countries. Turkey ranked 16th. 1351 (97.194%) of the publications were published in SCI-EXPANDED journals. The first article was published in the year 1971. 990 (71.87%) of the articles were published after 2000. The highest number of publications was in the year 2020 (4.892%). The H index of the articles was 100. There were a total of 45063 citations and an average of 32.42 per item. The number of citations and publications has increased over the last 30 years. The University of London was the top-ranked affiliation. Most of the articles (11.871%) were published in the journal "Transplantation Proceedings". Conclusions: The number of publications in this field is very low, especially for developing countries. The USA, Japan, and European countries are the countries with the highest number of publications.

This study of 331 primary brain abscess (PBA) patients aimed to understand infecting agents, predisposing factors, and outcomes, with a focus on factors affecting mortality. Data were collected from 39 centers across 16 countries between January 2010 and December 2022, and clinical, radiological, and microbiological findings, along with their impact on mortality, were analyzed. The patients had a mean ± SD age of 46.8 ± 16.3 years, with a male predominance of 71.6%. Common symptoms included headache (77.9%), fever (54.4%), and focal neurological deficits (53.5%). Gram-positive cocci were the predominant pathogens, with Viridans group streptococci identified as the most frequently isolated organisms. All patients received antimicrobial therapy and 71.6% underwent interventional therapies. The 42-day and 180-day survival rates were 91.9% and 86.1%, respectively. Significant predictors of 42-day mortality included intravenous drug addiction (HR: 6.02, 95% CI: 1.38-26.26), malignancy (HR: 3.61, 95% CI: 1.23-10.58), confusion (HR: 2.65, 95% CI: 1.19-5.88), and unidentified bacteria (HR: 4.68, 95% CI: 1.76-12.43). Significant predictors of 180-day mortality included malignancy (HR: 2.70, 95% CI: 1.07-6.81), confusion (HR: 2.14, 95% CI: 1.11-4.15), temporal lobe involvement (HR: 2.10, 95% CI: 1.08-4.08), and unidentified bacteria (HR: 3.02, 95% CI: 1.49-6.15). The risk of death in PBA extends beyond the infection phase, with different factors influencing the 42-day and 180-day mortality rates. Intravenous drug addiction was associated with early mortality, while temporal lobe involvement was associated with late mortality.

Background/Objectives: Urinary tract infections (UTIs) in older adults are common and potentially life-threatening conditions that often present with atypical symptoms. Early identification of prognostic factors is essential to improve clinical outcomes and reduce mortality in this vulnerable population. Methods: This retrospective, multicenter study included patients aged ≥65 years who were hospitalized with a diagnosis of UTI between January 2019 and December 2023. Diagnoses were established by infectious disease specialists based on clinical findings and microbiological confirmation in accordance with international guidelines. Only patients with urine cultures showing ≥100,000 CFU/mL bacterial growth were included. Demographic, clinical, laboratory, and microbiological data were analyzed. Multivariable logistic regression was used to identify independent predictors of in-hospital mortality. Results: A total of 1175 patients (median age: 75 years; 51.1% male) were included. The in-hospital mortality rate was 14.6%, and 25.6% required intensive care unit (ICU) admission. Multidrug-resistant (MDR) bacteria were detected in 63.6% of isolates, and bacteremia was present in 24.3% of cases. In multivariable analysis, MDR/ESBL positivity (OR: 2.09, 95% CI: 1.24–3.50, p = 0.005), bacteremia (OR: 2.10, 95% CI: 1.32–3.35, p = 0.002), and SOFA score (OR: 1.53, 95% CI: 1.42–1.65, p < 0.001) were independently associated with in-hospital mortality. Age and altered mental status were also significant predictors, while CRP and procalcitonin lost significance after adjustment. Conclusions: UTIs in elderly patients are associated with substantial morbidity and mortality. Multidrug-resistant pathogens, bacteremia, and disease severity play a central role in determining outcomes. Early identification of high-risk patients using clinical severity scores and microbiological data may improve risk stratification and guide timely, targeted therapeutic interventions.
