Background: The New Global Economy is represented by a series of major features, such as the use of green energy, the reduction of the carbon footprint in all industrial and civil fields, as well as finding alternative food resources. Our main objective was the research of a sustainable food product with a special nutritional purpose in the vision of nutrivigilance, developed in Romania, as an adjuvant in the repair of gastric mucosa. Methods: The materials used in the research and development of the new food are the following: inulin, lactoferrin, sericin, and sodium bicarbonate. The new adjuvant food product in the repair of the gastric mucosa was added to certain foods in order to prevent the patients from being satiated by a single food from a sensory point of view. The resulting food products were organoleptically and physico-chemically analyzed. Results: The new food is sustainable and has versatile uses. It can be hydrated with water, non-carbonated drinks, mixed with cottage cheese, or with fruit puree and oatmeal. It is stable under normal storage conditions and microbiologically safe. Conclusions: Through its versatile use, the new food product for special nutritional conditions represents a worldwide novelty. Through the development of forestry for the cultivation of white or black mulberry (Morus alba and Morus nigra), the raising of silkworms (Bombyx mori), the processing of fibroin to obtain natural silk and the processing of sericin resulting as a residue in the textile industry, the new food product developed actively contribute to the global economy II.

Textural analysis is pivotal in augmenting the diagnosis and outcomes of endovascular procedures for stroke patients. Due to the detection of changes imperceptible to the human eye, this type of analysis can potentially aid in deciding the optimal type of endovascular treatment. We included 40 patients who suffered from acute ischemic stroke caused by large vessel occlusion, and calculated 130 different textural features based on the non-enhanced CT scan using an open-source software (3D Slicer). Using chi-squared and Mann–Whitney tests and receiver operating characteristics analysis, we identified a total of 21 different textural parameters capable of predicting the outcome of thrombectomy (quantified as the mTICI score), with variable sensitivity (50–97.9%) and specificity (64.6–99.4%) rates. In conclusion, CT-based radiomics features are potential factors that can predict the outcome of thrombectomy in patients suffering from acute ischemic stroke, aiding in the decision between aspiration, mechanical, or combined thrombectomy procedure.

Background and Clinical Significance: Retroperitoneal tumors are a rare and diverse group of neoplasms, accounting for less than 1% of adult solid tumors. Retroperitoneal lipomas are particularly uncommon, with fewer than 20 cases described in the literature. Their asymptomatic growth and lack of clear anatomical boundaries can result in delayed diagnosis and substantial tumor size at clinical presentation. This case highlights a rare retroperitoneal lipoma with atypical extension into the right thigh through the muscular lacuna, mimicking a femoral hernia and compressing the femoral nerve—a presentation scarcely reported and clinically significant due to its surgical complexity and risk of recurrence. Case Presentation: We report the case of a 65-year-old woman from an urban setting who presented with progressive right thigh discomfort and lower limb pain during ambulation. The mass had been initially identified two years prior as a small, asymptomatic right inguinal formation during imaging to exclude an inguinal hernia. Computed tomography (CT) and magnetic resonance imaging (MRI) confirmed a large retroperitoneal lipomatous tumor extending to the anteromedial right thigh. Surgical excision was performed through a dual approach: midline laparotomy and thigh incision. A 30 × 30 cm encapsulated lipoma was removed without injuring adjacent nerves or vessels. Histopathological evaluation confirmed a mature lipoma without atypia but with a lipogranulomatous reaction. The patient’s postoperative course was favorable, with minimal residual paresthesia and complete wound healing. Conclusions: Although benign, retroperitoneal lipomas can mimic other pathologies and present surgical challenges when they extend beyond their typical boundaries. Early recognition and coordinated surgical management are crucial for optimal outcomes and prevention of recurrence.

Background/Objectives: Systemic inflammatory indices may differ prognostically between resectable and palliative pancreatic cancer. We compared associations of the aggregate index of systemic inflammation (AISI), systemic inflammation response index (SIRI), neutrophil-to-lymphocyte ratio (NLR), systemic immune-inflammation index (SII), and monocyte-to-lymphocyte ratio (MLR) with overall survival after pancreatic resection or palliative bypass. Methods: This single-center retrospective cohort included 240 patients treated during 2013–2023 (99 resections; 141 palliative bypasses). Indices derived from preoperative blood counts were natural-log transformed, standardized to 1 SD, and entered separately into operative-group-stratified Cox models. Group-specific associations and interactions were estimated. Reconstructed AJCC eighth-edition stage was incorporated only in the resection stratum. Interaction p values were Holm-adjusted. AISI was designated primary and SIRI key secondary. Results: Overall, 215 deaths occurred. Median overall survival was 17.9 months after resection and 5.4 months after palliative bypass. AISI was associated with mortality following bypass (adjusted hazard ratio [HR] per 1-SD increase in ln(AISI), 1.54; 95% confidence interval [CI], 1.28–1.86) but not following resection (HR, 0.97; 95% CI, 0.75–1.25; interaction p = 0.003; Holm-adjusted p = 0.010). SIRI showed corresponding HRs of 1.60 (95% CI, 1.30–1.96) and 0.93 (95% CI, 0.72–1.20), respectively (interaction p = 0.001; Holm-adjusted p = 0.004). Exploratory MLR analysis also showed statistical heterogeneity. SII was associated with mortality following bypass, but its operative-group interaction was not significant. The conventional NLR interaction was also nonsignificant, although model-assumption violations limited its interpretation. The AISI and SIRI findings remained robust in the sensitivity analyses. Conclusions: AISI and SIRI showed statistical heterogeneity in their associations with overall survival between operative groups, with higher values associated with mortality following palliative bypass. These findings do not establish that the operative procedure modifies the underlying biological relationship. The absence of pathological staging in the bypass group leaves potential residual confounding by disease extent. Independent external validation is required before clinical implementation.

We compared associations of the aggregate index of systemic inflammation (AISI), systemic inflammation response index (SIRI), neutrophil-to-lymphocyte ratio (NLR), systemic immune-inflammation index (SII), and monocyte-to-lymphocyte ratio (MLR) with overall survival after pan-creatic resection or palliative bypass. Methods: This single-center retrospective cohort included 240 patients treated during 2013–2023 (99 resections; 141 palliative bypasses). MLR also demonstrated heterogeneity, whereas NLR and SII did not. The AISI and SIRI findings remained robust in sensitivity analyses. Conclusions: AISI and SIRI demonstrated operative-context-dependent associations, principally after palliative bypass. Because comparable staging was unavailable in this group, residual confounding by disease extent remains possible. Prospective multicenter validation is required before clinical implementation.

Introduction Osteoporosis is characterized by a reduction in bone mineral density. Among the factors that can contribute to the onset of osteoporosis we can enumerate alcohol consumption, smoking, glucocorticoid therapy, or the presence of diabetes mellitus. The incidence of osteoporosis increases with age. Materials and Methods A total of 183 women over the age of 50, with a mean age of 67.9 ± 7.74 years, were studied to visualize spinal column alterations. From this cohort, 103 patients underwent bone mineral density testing using dual-energy X-ray absorptiometry (DXA) and dorsolumbar radiography, while 80 patients did not undergo DXA testing. Assessment of the degree of vertebral involvement was performed using the Genant semiquantitative method. Results The highest percentage of vertebral fractures was observed in patients aged >70 years (100%). Within the studied cohort, 28% of patients displayed multiple vertebral fractures associated with age (p = 0.01). The most common site for vertebral involvement was the dorsolumbar region (D11–L2) across all age groups (p = 0.35). No statistically significant correlation (p = 0.22) was identified between DXA values and the presence of vertebral fractures, despite a trend of increased incidence of fractures as the T-score decreased. Conclusion Within the cohort, vertebral fractures were identified both in women with normal values
