Up-to-date data on patients with chronic obstructive pulmonary disease (COPD) in the Russian Federation are necessary to improve medical care effectiveness. This article presents the results of the retrospective part and the cross-sectional assessment of the CORSAIR study, which aimed to assess the distribution of COPD patients in the Russian population by symptom severity and exacerbation risk as per GOLD (2020) classification groups A, B, C, D.Methods. The observational multicenter study CORSAIR included a cross-sectional assessment with data collection during the previous 12 months (retrospective part) and subsequent follow-up for 12 months (prospective part). Data from 704 patients obtained at 18 study sites from August 2021 to November 2022 are presented. At the first visit, the physician recorded medical history, clinical data on the disease course, and COPD therapy, assessed compliance of treatment with national guidelines, and determined whether treatment change was needed considering the predominant treatable trait (dyspnea or exacerbation).Results. Upon inclusion, most patients had severe COPD symptoms (mMRC score ≥ 2; САТ score ≥ 10) and severe and very severe airflow obstruction (GOLD III and GOLD IV; FEV1 < 50% оf predicted). More than half of the patients had at least one moderate or severe COPD exacerbation within the previous 12 months. As per the GOLD (2020) classification, 57.2% of patients belonged to Group B (severe symptoms and low risk of exacerbations) and 30.3% to Group D (severe symptoms and high risk of exacerbations). 58.8% of patients received treatment that was not compliant with national clinical guidelines in force at the study initiation. 31.7% of patients had not COPD control. Blood eosinophil count was above 300 cells/μL in 15.1% of patients.Conclusion. In most cases, patients had severe COPD symptoms with frequent exacerbations, and the prescribed treatment did not comply with national clinical guidelines. These data will be analyzed alongside the prospective study results.

Over the past few decades, various applications of the metabolic drugs have been extensively tested. Most of them affect oxygen-dependent processes, improving cellular metabolism and increasing tissue resistance to hypoxia and ischemia. The most promising candidates include components of the respiratory chain, purine nucleosides, and creatine phosphate which affect glucose oxidation and fatty acid metabolism in the Krebs cycle. This review critically evaluates the most popular drugs of this group (adenosine triphosphate, adenosine-5-monophosphate, creatine phosphate, coenzyme Q10, cytochrome C, adenosine, glucose-insulin-potassium solution, L-carnitine, mildronate, and trimetazidine), which are widely represented on the pharmaceutical market. Of all metabolic drugs, only trimetazidine was included in the European and Russian recommendations for the second-line treatment of stable angina. In most clinical studies, the therapeutic efficacy of metabolic drugs has been evaluated using the surrogate endpoints. Despite being actively advertised and widely used in the clinical practice, metabolic drugs currently do not have a convincing evidence base for affecting prognosis (mortality and/or major adverse cardiovascular events). Further studies in large-scale randomised trials are needed to confirm the beneficial effects of the metabolic drugs in cardiovascular medicine.

Aim. Development of the chronic periodontitis treatment method using the medicamentation based on herbal components in the experiment.Materials and methods. An experimental model of chronic periodontitis was created. Mesenchymal stem cells (MSC) from the extracted retained human tooth were obtained multipotent. The obtained MSCs were injected into the periodontalpockets together with the osteogenesis stimulator drug. Then a medicamentation based on plant components was applied.Results. Periodontal pocket depth decreased to 1.5 ± 0.01 mm (P < 0.001). There was no tooth mobility. Three months after the therapeutic procedures radiographs of the rats showed signs of bone neoplasia. By 6 months these signs became more pronounced: the foci of stained osteoporosis disappeared and began to acquire a finely petrified structure, the contours of the interalveolar septa became clear and smooth with a simultaneous increase in the height of resorbed ridges of the interalveolar septa.Conclusions. Experimental studies have shown pronounced therapeutic efficacy of the method for the treatment of destructive changes of the alveolar bone in periodontitis.

RELEVANCE: An artificial gum recession defect was created in 50 Wistar rats. SANS MOTS oil containing marque blanche phytoconcentrate with phytosterols was applied to the defect area for 20 min. 5 times, with a 2-day interval. AIM: To evaluate the effectiveness of the treatment method. MATERIAL AND METHODS: An experimental gum recession model was created by mechanically excising periodontal tissues in a V-shape. Fifty white female rats were used as the model. Surgical intervention was performed on the lower jaw in the incisor area. The animals were divided into 2 groups depending on the planned treatment method. The control group consisted of 10 rats with healthy gums. The SANS MOTS oil was applied to the defect area for 20 min, 5 times, with an interval of 2 days in the other group. The effectiveness of the treatment was evaluated by the proposed method. RESULTS: Complete restoration of the dentoalveolar papillae and regeneration of periodontal tissues occurred in the experimental gum recession area during the first 28 days. At the same time, the main group developed no signs of inflammation (hyperemia, edema, bleeding gums), which confirms the therapeutic effect of the SANS MOTS oil. CONCLUSION: The SANS MOTS oil had a beneficial effect on gum recession. The decrease in the intensity and prevalence of gingival inflammation began on day 14. There were no signs of inflammation by day 28 after applying the oil. In addition, there was a noticeable increase in regeneration of the gingival margin by day 24 in the gum recession model, and complete restoration of the gingival papillae and no gum recession were noted by day 38.

PURPOSE. Improving the results of the operation and achieving its reliability in secondary glaucoma, including refractory glaucoma, can be done using a non-biodestructible allograft drainage with spongy structure fabricated by Alloplant® technology.METHODS. Experimental and morphological studies conducted in the Russian Eye and Plastic Surgery Center showed that the allogeneic spongy biomaterial used for anti-glaucoma sponge drainage surgery is gradually replaced by loose tissue with a porous structure, similar to trabecular, forming a new drainage system. During the operation, the proximal part of the drainage is inserted through the excised drainage zone in the anterior chamber angle (ACA). The distal part is tucked into the suprachoroidal space through a linear incision in the scleral bed.RESULTS. The newly created topography of the ACA after sponge drainage operation with the use of allogeneic biomaterial features not only the expansion of the ACA profile, but also the replacement of the excised part of the drainage zone with biomaterial after sinus trabeculectomy (STE) and elimination of retention of the intraocular fluid (IOF) outflow. Ultrasound biomicroscopy (UBM) performed at various times after antiglaucoma surgery showed that the microtubule system preserved. Optical coherence tomography (OCT) of the ACA demonstrates the location of the drainage in the long-term follow-up.CONCLUSION. Antiglaucoma operation with the use of allogeneic biomaterial with spongy structure promotes the formation of trabecula-like tissue in the ACA, eliminates retention of IOF outflow, and reduces the risk of scleroscleral and sclero-conjunctival adhesions.

Introduction. It is known that the problem of regeneration of damaged peripheral nerves is one of the leading problems in traumatology and neurosurgery.Aim. Aim of the study is to determine the features of the innervation of facial muscles when using allogeneic biomaterials.Materials and methods. The experiment was performed on Chinchilla rabbits (n = 36). The facial nerve was cut in the animals. In the control group (n = 9), the wound was sewn up, in the 1st experimental group (n = 12), a fragment of their masticatory muscle was sewn together with a neurovascular bundle to the denervated buccal muscle. In the 2nd experimental group (n = 15), dispersed allogeneic biomaterials "Regeneration stimulator" and "Vasculogenesis stimulator" were injected into the muscle junction in the operation zone. Rabbits were withdrawn from the experiment for 10, 30, 60 and 180 days. Tissue pieces from the surgery area were examined by transmission electron microscopy. Results and discussion. In the control and 1st experimental groups, the experiment ended with scarring of the operating area and contracture of facial muscles. In the 2nd experimental group, signs of tissue revascularization and axon germination to the buccal muscle with the restoration of individual neuromuscular synapses were revealed.Conclusion. The use of allogeneic biomaterials in operations to restore damaged peripheral nerves accompanying muscles creates conditions not only for the restoration of muscle fibers, but also the vascular bed, as well as nerve elements with neuromuscular connections.
