Background: Age-related declines in respiratory muscle strength and ventilatory efficiency can impair functional capacity and metabolic health in older adults. Inspiratory muscle training (IMT) has been proposed as a practical intervention to counteract these changes, yet its systemic effects remain unclear. This study aimed to examine the effects of short-term IMT on functional capacity, diaphragm thickness, and liver tissue characteristics in healthy elderly men. Methods: Thirty community-dwelling men aged 60–80 years were randomly assigned to an IMT or control group. The IMT group performed four weeks of breathing exercises using a POWERbreathe® device at 40% of maximal inspiratory pressure, with a weekly 10% increase in pressure. Pre- and post-intervention assessments included the six-minute walk test (6MWT), diaphragm thickness and liver density via computed tomography, and quality of life (QoL; SF-12). Results: Four weeks of inspiratory muscle training significantly improved diaphragm thickness (11.7%), fatty liver density (FLD) (+16.7%), and six-minute walk performance (+5.3%), with large time × group effects favoring the IMT group. While the physical quality of life showed modest, comparable improvements, mental health outcomes demonstrated a moderate, time-dependent improvement without a significant group-by-time interaction. Conclusions: Short-term IMT improved diaphragmatic function and functional capacity in older men and was associated with favorable changes in a liver-related biomarker; however, given that only a single liver-related metric was assessed, these findings should not be interpreted as evidence of overall improvements in liver health.

Handball requires athletes to sustain intermittent high-intensity effort while maintaining rapid cognitive processing and technical skills. Caffeine is widely used as an ergogenic aid, yet its dose-dependent effects across physical, cognitive, and technical performance outcomes remain unclear in female handball players. This randomized, double-blind, placebo-controlled crossover study examined the acute effects of low-dose caffeine (LCAF; 3 mg/kg), moderate-dose caffeine (MCAF; 6 mg/kg), and placebo (PLA) in trained female handball players. Participants (n = 20) completed three experimental sessions separated by 72 h. Cognitive performance was assessed using the Simplified Eriksen Flanker Test, throwing performance was evaluated through maximal ball velocity, and intermittent running capacity was measured with the Yo-Yo Intermittent Recovery Test Level 1. Both LCAF and MCAF significantly improved Yo-Yo performance compared with PLA (η2p = 0.415, representing improvements of approximately ~23.5% and 29.0%), with no difference between caffeine doses. MCAF significantly reduced overall Flanker response time (η2p = 0.486, ~18.5%) and congruent and incongruent trial response time compared with PLA and LCAF. No significant effects were observed for throwing velocity, Flanker accuracy and interference scores. These findings suggest that acute caffeine intake has performance-specific effects in female handball players: intermittent running performance responded to both doses, whereas cognitive enhancement was limited to response time, with no improvements in inhibitory control or accuracy.

Although caffeine is widely used in athletes due to its ergogenic effects, the effects of its main metabolite, paraxanthine, on performance and sleep have not been adequately investigated. This study aimed to comparatively investigate the effects of caffeine and its main metabolite paraxanthine on rowing performance and sleep quality. The study was designed as a randomized, double-blind, crossover study and included 14 male university-level rowers (21.6 ± 1.9 age; 2.2 ± 1 years of rowing experience). The participants participated in 2000-m rowing ergometer time trials under four different supplementation conditions (caffeine + paraxanthine, caffeine + placebo, paraxanthine + placebo, and placebo with 200 mg each). Performance data (completion time, mean power, and heart rate), sleep quality, and daytime sleepiness were assessed by subjective scales. The data were analyzed by repeated-measures analysis of variance (ANOVA) and Bonferroni post-hoc tests. Compared with the placebo, the combined caffeine + paraxanthine condition was associated with faster 2000-m performance and higher mean power output (p = 0.044; Cohen's d = 0.30). Caffeine alone and paraxanthine alone did not show clear evidence of performance improvement in this sample, although estimates favored both conditions versus placebo. Conditions containing caffeine were associated with poorer subjective sleep quality, whereas paraxanthine alone showed more favorable sleep-related outcomes. These results indicate that the combined ingestion of caffeine and paraxanthine elicited the most pronounced performance benefits, while paraxanthine alone did not demonstrate clear standalone ergogenic efficacy at the administered dose. However, paraxanthine was associated with better subjective sleep outcomes compared to caffeine, suggesting that its potential value may be related more to tolerability rather than superior performance enhancement, particularly for athletes training in the evening. Study limitations, including the small sample size and lack of objective sleep measures, should be considered when interpreting the results. Paraxanthine may represent a practical option for athletes who prioritize sleep quality or experience sensitivity to caffeine-related sleep disturbances, although further dose‒response studies are required to clarify its ergogenic potential.

Background: Thoracic hyperkyphosis (THK) is a postural deformity associated with pain, impaired respiratory function, reduced quality of life, scapular rhythm impairment, shoulder biomechanical alterations, and increased fall risk. While corrective exercises are commonly used, the added value of integrating specific respiratory techniques remains unclear. This systematic review and meta-analysis aimed to evaluate the effects of movement-based exercises combined with respiratory techniques on the thoracic kyphosis angle (TKA) in individuals with THK. Methods: A systematic search of PubMed, Scopus, Web of Science, CINAHL, SPORTDiscus, PEDro, and CENTRAL Cochrane Library was conducted from inception to 16 July 2026. Randomized controlled trials (RCTs) and non-randomized controlled trials (non-RCTs) investigating combined exercise and respiratory interventions in individuals with THK were included. The primary outcome was TKA. Two authors independently selected studies, extracted data, and assessed quality, with a third author resolving discrepancies. Risk of bias was assessed using RoB-2 for RCTs and ROBINS-I for non-RCTs. Certainty of evidence was graded using GRADE. A random-effects meta-analysis of TKA was performed using Hedges’ g. Results: Five studies involving 247 participants were included. Meta-analysis demonstrated a significant reduction in TKA following combined intervention compared with exercise alone (Hedges’ g = 0.667, 95% CI: 0.395 to 0.940, p < 0.001; I2 = 39.7%). Subgroup analysis by age group revealed significant improvement in adolescents (Hedges’ g = 0.860, 95% CI: 0.532 to 1.189, p < 0.001; I2 = 0%) but no significant effect in older adults (Hedges’ g = 0.241, 95% CI: −0.247 to 0.729, p = 0.333; I2 = 0%). The certainty of evidence for TKA was graded as moderate, with downgrading for risk of bias, indirectness, and potential publication bias. Conclusions: Movement-based exercises combined with respiratory techniques significantly improve thoracic alignment in individuals with THK, with moderate certainty evidence, particularly in adolescents. The evidence in older adults remains inconclusive due to limited studies and structural factors. Larger, high-quality, and standardized trials are needed to confirm these effects, establish optimal treatment parameters, and evaluate long-term outcomes across diverse populations.
