The implementation of CAD-CAM systems in dentistry has significantly influenced the evolution of dental implantology and implant-supported prosthetics within the past three decades. Implant-supported prostheses are comfortable and aesthetic. The prosthetic abutment has also faced a rapid design evolution, from the individualization of standard stock abutments offered by various manufacturers to a modern customization process using CAD-CAM technology. This paper presents a comparative study between 20 dental custom CAD-CAM implant abutments and 20 dental implant stock abutments, based on a set of measurements performed on the digital casts obtained from 24 cases of prosthetic rehabilitation on implants. The statistical analysis (Mann–Whitney U test) revealed significant differences between these two types of abutments: the incisal margin line diameter dimensions for custom abutments were significantly improved compared to standard abutments at the cervical level (U = 343.00, z = 3.868, p < 0.0005) and the incisal/occlusal level (U = 352.00, z = 4.112, p < 0.0005), while the inclination angle of the custom abutments relative to the 0-axis was significantly smaller than that of standard abutments (U = 115.50, z = −2.286, p = 0.022). The use of custom abutments leads to an increase in the final size of the abutment, an improvement in the retention of the prosthetic work, and reduces the angulation of the abutment in relation to the implant axis, thus decreasing the risk of unscrewing or fracturing the dental screw.

Introduction: Dental occlusion refers to the static and dynamic relationships that are established between the teeth of the two arches and is an important factor in the homeostasis of the dento-maxillary system. The objective of the present study was to compare two digital occlusal analysis systems: the T-Scan III system and the Medit I600 intraoral scanner. Materials and Methods: The study was carried out on 20 students from the Faculty of Dental Medicine Craiova, whose dental occlusion was assessed with the T-Scan III system and with the Medit I600 intraoral scanner. Dental occlusion was assessed in the maximum intercuspation position, the edge-to-edge protrusion position, and the edge-to-edge position in right and left laterotrusion. The images of the 2D occlusal contact areas obtained by both methods were converted to .jpeg format and then transferred to Adobe Photoshop CS6 2021 (Adobe Systems, San Jose, CA, USA) for comparison. The recorded data were statistically processed. Results: Analyzing the data provided by the two digital occlusal analysis systems, it was found that the T-Scan III system provided data related to the amplitude of the occlusal forces, the surface on which they were distributed (the contact surface), the dynamics of the occlusal contacts, and the proportion in which they were distributed at the level of the two hemiarches, and the Medit I600 intraoral scanner performed an evaluation of the occlusal interface of the two arches, highlighting the extent of the contact areas with the degree of overlapping of the occlusal components. Although both methods of occlusal analysis recorded the highest values for the maximum intercuspation position, the results could not be compared. Conclusions: The two digital systems provide different data in occlusal analysis. As the T-Scan III system is considered the gold standard for occlusal analysis, more studies are needed to understand the data provided by the Medit I600 intraoral scanner and their significance.

Currently, there are several digital technologies for the manufacture of occlusal appliances used in bruxism, which may involve subtractive technologies or additive technologies. The purpose of the study is the presentation of a digital workflow for occlusal splint through CAD-CAM with 3D printing. Material and method: The technology used for the manufacture of the occlusal splint included as stages: obtaining digital models, creating the digital design of the splint, 3D printing of the splint from a rigid PMMA material, adapting the splint on dental arch and in occlusion. Results: The digital technology for the occlusal appliance was used for a patient with sleep bruxism. The patient wore the occlusal splint for 9 weeks, with positive feedback. Conclusions: The digital technology proposed for occlusal splints in bruxism is reliable, and the rigid PMMA occlusal splints 3D printed are a viable alternative to conventional occlusal appliances in bruxism.

Personalized medicine has become an important direction to offer better solutions for health problems. In implantology, this trend was materialized through customizing dental abutments to each clinical situation. The demands for better esthetics and function of implant-supported restorations have imposed a more personalized variety of prosthetic abutments. This retrospective study compared clinical efficiency of personalized implant abutments with standard implant abutments in multiple implant restorations. Clinical data of patients who were admitted in a private clinic between 2011 and 2022 and received dental implant treatments were collected. All complications and undesired events from the patients’ medical record charts were statistically analyzed. The implants were loaded using either standard or customized abutments. For complete arch rehabilitations with the SKY Fast & Fixed protocol, standard titanium prosthetic abutments were used. Our results suggest that the abutments choice for patients has moved throughout the years more towards the use of customized abutments. The number of customized abutments (414) was higher compared with the number of standard abutments (293). In our database, the most used abutments for the anterior area implants were made of titanium and zirconia, whereas for the posterior area, the preferred abutments were mostly titanium. The standard abutments were used almost entirely for immediate loading and implantation in both anterior and posterior areas (Fast & Fixed protocol). Complications were encountered mainly in restorations with standard abutments (9.22%) compared to customized abutments (2.7%), with titanium abutments being the most reliable, having only 1.79% complications.

Background/Objectives: Bruxism is a masticatory muscle activity, phasic or tonic, with/without teeth contact, that appears in sleep or an awake state. An instrumental technique used to measure the surface electromyographic (sEMG) activity of the masseter muscle is used to diagnose bruxism activity during sleep and while awake. The objective of this study was to compare the variation in bruxism (sleep and awake) indices and masseter activity indices in low sleep bruxism and moderate sleep bruxism before and after wearing an occlusal appliance (OA) for 3 months each night. Methods: A clinical interventional study was designed in which subjects diagnosed with sleep bruxism were randomly selected to be included in the study. After the first sEMG recording, two groups were formed: a low sleep-bruxism group (number of sleep-bruxism events/h between 2 and 4) and a moderate sleep-bruxism group (number of sleep-bruxism events/h equal or higher than 4). All subjects received treatment with a 3D-printed occlusal appliance and wore it each night for 3 months, at which point the second sEMG recording was performed. For each participant of this study, a chart was created that included anamnestic data, clinical data, and sEMG data. The data were statistically analyzed with SPSS, using the Mann–Whitney U and Wilcoxon signed-rank tests. Results: A total of 21 participants were included in the final analysis, 18 women and 3 men, with a mean age of 24.5 ± 2.7 years. The OA lowered all bruxism indices in the whole group, but clusters analysis showed a significant reduction in sleep-bruxism indices in the moderate sleep-bruxism group, while in the low-bruxism group, the sleep and awake indices varied insignificantly, and the number of sleep-bruxism events/h remained constant. Conclusions: The 3D-printed occlusal appliances significantly lowered the sleep-bruxism indices and sleep masseter activity indices recorded with a portable sEMG device in the moderate sleep-bruxism group. The OA lowered the awake-bruxism indices and awake masseter activity indices in the moderate sleep-bruxism group.

This retrospective study aimed to evaluate the frequency of dental materials use for fixed prosthesis depending on the location of the teeth, the restorative prosthetic type, the age and sex of the patients from a dental practice in Essen, Germany. The analysis of the collected data showed that zirconia is currently the most common material for making dental fixed prostheses, a larger number of prosthetic elements for women and a significant increase for these restorations at ages over 40 years. Most of the prosthetic elements were made in the mandibular molar area, with an increased frequency of zirconia bridges in the mandible, while for the upper jaw the number of crowns was higher. Zirconia was more frequent used in female patients and in the age group 60-69.
